Main duties of the job As a Staff Nurse you will be responsible for providing care topeople admitted to our ward for rehabilitation.
We work closely as amulti-disciplinary team.
Our focus is promoting the comfort andwellbeing of patients, enabling them to live as independently aspossible.

As a team we encourage ongoing learning and development throughreflection and clinical supervision which we try and embed in our dailypractice.
Our nurses are required to facilitate the day to day running of theward, administering medications, working alongside all members of the MDT aswell as patients, their families, and carers to provided 24/7 rehabilitation toenable the highest level of independence and quality of life.
Our nurses arerequired to attend meetings as needed such as MDT and CHC or DischargePlanning.

Practiceas a member of the Community Hospital Nursing team, to support and work inpartnership with the wider multidisciplinary team.Planning and Delivery ofIndividual Nursing Care under Direction of Senior Staff Work within the NMC Code ofProfessional Conduct.Assess, plan, and implement andevaluate patient focused nursing care.Document care given andmaintain contemporaneous records.Maintain records in accordancewith the Trusts policy and that of NMC Code of Professional Conduct.Assist in implementation ofhealth promotion strategies, as agreed by Ward Manager.In consultation with seniorqualified staff, provide and monitor patient centred care within Trust agreedstandards.In collaboration with otherteam members organise the reception and discharge of patients ensuringeffective communication in transfer of information.Carry out delegated nursingcare.Use and participate in research/ audit in care delivery.Adhere to and promote allclinical practice, health and safety and corporate Trust Policies andGuidelines.For fulldetails of the duties and criteria for the role please refer to the jobdescription and person specification attached.
Person Specification Knowledge Essential Confidence to teach student nurses, patients, carers and members of the multidisciplinary team Understanding of evidence-based nursing, multidisciplinary working, clinical governance Awareness of Audit Evidence of on-going continuing professional development Desirable Medication administration via various routes PO, PR, S/C, IM, IV Experience of Audit Knowledge and understanding of Mental Capacity and DOLs Experience Essential Experience gained during pre-registration training Desirable Experience gained working in the frailty care setting.
Qualifications Essential RGN/RMN NMC Registered I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Desirable Practice Supervisor and/or Practice Assessor Training.
