Job summary TheCommunity Inpatient Unit provides Rehabilitation, Re-ablement and recoveryinterventions for adults in North East Lincolnshire.
The service will promotefaster recovery from illness, prevent unnecessary acute hospital admissions andpremature admissions to long-term care, support timely discharge from hospitaland maximise independent living wherever possible.
Multi-professional staffwill collaborate with individuals to ensure they achieve their full potentialin terms of their level of independence, health, and wellbeing.Please note that Care Plus Group are not currentlyregistered to sponsor applicants from outside the EU and EEA.
Main duties of the job Job title: Staff NurseDepartment: Community Inpatient UnitBase: Community Impatient Unit (Cambridge Park)Hours: hoursFixed Term for 9 Months to cover Maternity LeaveSalary Range 5 £29,734 - £34,474per annumWe are looking for a Staff Nurse to join us, you will bepart of a multidisciplinary team providing nursing and enablement services to patientswithin the rehab and recovery inpatient unit.Therole will require you to undertake various clinical duties includingassessment, wound management, injections, and continence care aswell as dealingwith acutely unwell.
Ideally we arelooking for some with previous community experience however this is notessential as you will received training and support.
Newly qualified will be considered as youwill be supported through a robust preceptorship programme.
Inreturn you will be part of a forward thinking MDT team, receive opportunitiesfor further development, regular supervision and support.

Person Specification Experience Essential Demonstrate evidence based practice Commitment to Lifelong Learning Experience of working with multidisciplinary team Experience of delivering a range of clinical interventions Experience of assessing rapidly deteriorating patients Desirable Research experience Mentorship/Teaching Experience Qualifications Essential Registered Nurse Recognised mentor qualification and/or Live on the mentors register OR willingness to work towards Desirable Any post registration nursing qualification Community Formulary Prescribing, Tissue Viability Knowledge & Skills Essential Ability to maintain clear and accurate records Ability to communicate complex issues Ability to communicate information to clients/carers with sensory impairment, learning disabilities, dementia and challenging behaviour.
Effective negotiation skills History of multi-disciplinary working and ability to liaise with other agencies Effective and accurate computer skills Ability to work on own initiative and as part of a team Ability to provide appropriate nursing care by prioritising and effectively managing an unpredictable workload Understanding of patient escalation Understanding of holistic needs in relation to both Health & Social Care needs, acute and long term conditions Ability to manage own workload Desirable Supervisory experience Trained in administration of Intravenous medications Extended clinical skills
