Job summary An exciting opportunity for a Band 5 Registered Nurse has arisen on our Discharge Lounge.
The post holder must be registered with NMC UK registration.We are looking for an enthusiastic nurse who can work dynamically and at a fast pace to ensure safe and effective care delivery through the Discharge Lounge.
As a member of our team you will work closely with patients, their families and the multi-disciplinary team to ensure that high quality care is delivered and a positive experience for patients through the Discharge Lounge.
Main duties of the job Using clinical knowledge/expertise,provide a high standard of skilled careto patients based on a full assessmentof the individuals needs in consultationwith the multi-professional team.To devise and evaluate patient careplans ensuring that realistic goalsare set in consultation with thepatient, their family and themulti-professional team.To ensure due regard is given tothe customs and spiritual beliefsof patients, carers and relatives.To promote and role model effectivecommunication within the multi-professionalteam.To act as a link nurse.To identify the learning needs ofpatients, relatives and staff and,in consultation with the Sister/ TeamLeader, plan and implement programmes.To ensure all documentation adheresto the Trust's documentationstandards.To ensure evidence-based practice isdelivered within the clinical areathrough participating in research orfacilitating the application of researchfindings into practice.

Person Specification Qualifications Essential Current registration with NMC on part one or two of the register.
Newly Qualified or working towards registering with the NMC.
Desirable Teaching and assessing qualification.
Other post-basic, health-related courses evidence of continuing professional development and its application.
Experience Essential Proven clinical practice in an acute hospital setting.
Experience of utilising reflective practice.
Desirable Experience of mentoring junior staff.
Experience of teaching junior staff.
Experience of shift: shift ward management in absence of senior colleagues.
Experience of clinical supervision.
Knowledge Essential Understanding of NMC Code of Conduct and guidelines of professional practice.
Good theoretical and practical knowledge of nursing within the clinical area.
Health and Safety awareness.
Understanding and knowledge of Quality issues and Clinical Governance.
