Job summary The post holder will be a key member of the Adult Speech and Language Therapy Team (ASLT) and Care Home Support Team (CHST).
CHST team includes a Pharmacist, Registered General Nurses, Registered Mental Health Nurses, Palliative care specialist nurse and Therapists.
Based at Upton house in Slough you will be a team player within the East Berkshire locality.
The service operates Monday to Friday, 9-5.
This is a hours part time band 4 permanent role.
Main duties of the job To provide Dysphagia training programmes to care home staff To audit implementation of training in care homes and adapt training packages as appropriate.
To request and collect data on the set observations carried out in the care homes as part of the audit information required for this service.
To provide guidance to care homes on ASLT referral processes and send relevant materials.
To create necessary resources to support care home staff to follow dysphagia and oral hygiene recommendations To promote the use of Dysphagia screening using triage screening assessment.
To advise on processes to monitor client to support SLT assessments oral hygiene logs and food diaries.
