Job summary The post holder will be employed as a member of the Psychiatric Liaison Service to provide a flexible resource to in-patient wards within the Princess of Wales Hospital also to in-Reach Care Home settings across the Bridgend County.
Main duties of the job Be a fully engaged member of the Psychiatric Liaison Service, providing a flexible resource within a defined area, supporting Liaison Practitioners with patients who may have complex needs in hospital and Care Homes.Undertake intervention programmes with individual patients as directed by a multi-disciplinary liaison team member, underpinned by appropriate experience and/or knowledge.Supervise and instruct patients' activities relating to their treatment programme, use of specific rehabilitation techniques and skills to assist in the provision of flexible, patient centred programme of therapeutic activities.Educate ward staff to address the needs of the patients who present with expressive behaviours that challenge Staff and have complex relational needs.Deliver direct patient care and implementation of strategies designed to reduce the risk and acuity presented by an individual.Restore and maintain functional ability for individuals within the hospital and care home setting single handily.Maintain clinical records (paper & electronic) and report accurately to others.Carry out administrative duties in relation to clinical, service, departmental needs.Liaise with patient families with respect to education and community services.
