Main duties of the job The team youwill work with consists of an extensive MDT which includes nurse practitioners,occupational therapists, physiotherapists, health care assistants, therapyassistant/exercise practitioners and administrative staff.
The team worksclosely alongside partner agencies including those from the local and voluntarysector.
This is a significant strength to our team we are proud of and supportspatient centred, integrated working.The teamdelivers planned and unplanned care to people in their own homes, to assist inpreventing hospital admission and supporting those who have recently beendischarged from hospital, require enhanced intervention and short termrehabilitation support for their physical and functional health needs to remainat home.
The service also covers rotation into Borrowdale House, a reablementunit.The service isfast paced and you would be expected to see a wide range of patients whopresent with a variety of conditions when supporting the team clinically.The service iscommitted to providing a sound environment for education and training tosupport the successful candidate within this role.This role wouldinvolve working predominantly core hours of Monday to Sunday 08:00 am 20:00 pmwith flexibility to the needs of the service.
We offer flexible working withinour service with a variety of shift patterns and flexibility to work until 8pmand to cover weekends.Candidates musthave a valid UK Driving Licence.
