Job summary East Coast Community Healthcare (ECCH) are looking for an enthusiastic, highly skilled senior physiotherapist with excellent communication skills to work as a Specialist Physiotherapist within our MSK Assessment and Triage (MATS) Team.The successful candidate will be offered training and mentoring from our experienced Extended Scope Practitioner's (ESPs) to meet the demands of the role.
ECCH MSK covers a large geographical area including Great Yarmouth and Waveney and North Norfolk, and we are part of NoW MSK, who provide community MSK services across Norfolk and Waveney.The successful candidate would be expected to work across all MSK hubs as required to meet the needs of the service.East Coast Community Healthcare CIC (ECCH) is entering exciting new times with integrated health and social care provision.
In partnership with local health and care providers we are evolving into a multidisciplinary, multi-organisational workforce as we continue to deliver community care.
There is an expectation that as this new system develops, this role will also evolve and change providing an exciting opportunity for the post-holder to further develop their skills and knowledge.For further information please contact Adi Payne, Clinical Lead ESP, via email- Main duties of the job - To provide Specialist triage, assessment and treatment where appropriate for patients with complex musculoskeletal (MSK), chronic pain and rheumatological conditions to ensure progress through the appropriate care pathway.
Including patients referred to secondary care.- To act as a source of expertise and advice on the Physiotherapy assessment and management of MSK dysfunction; providing advice to patients, senior Physiotherapists, other healthcare professionals, GPs and consultants.- To assist the ESPs for the MSK Assessment and Triage Service in the running of the Service.- To develop and maintain skills required to practice at an advanced level and to use these skills to aid diagnosis and develop treatment plans within individual clinical competencies which may include; - Joint/ soft tissue injections - Imaging /pathology request and interpretation About us ECCH is a well established health care provider and has been successfully delivering NHS care within the community since 2011.

Job description Job responsibilities ClinicalTo have an active role inthe assessment of a wide range of Musculo-skeletal, chronic pain andrheumatologically conditions based on advanced theoretical knowledge ofanatomy, physiology, neuroanatomy, and pathology, often in situations whereconflicting evidence and/or complex bio-psychosocial issues are present.
Toapply advanced clinical reasoning skills to assess and form a diagnosis andensure progress through the appropriate care pathway.To recognise the mostappropriate pathway for the patient such as when referral to specialistOrthopaedic, Rheumatology or Pain Management is fully appropriate.
To usetheoretical and practical knowledge to make recommendations for furthermanagement of the condition and assist the patient as far as possible in thereferral process.
This requires knowledge and understanding of the role ofother health professionals, ( psychology, occupational therapy, pharmacy,orthopaedics, rheumatology, pain clinic, general practitioner).To communicate some highlycomplex patient related information effectively to ensure collaborative workingwithin the MSK, pain and rheumatology services/teams and with multidisciplinarycolleagues across health and social care sectors to ensure the delivery of acoordinated multidisciplinary care.
Making appropriate recommendations forfurther management and ensuring that the patient is informed.
To accept clinicalresponsibility for a designated caseload of patients and to organise thiseffectively and efficiently with regards to clinical priorities and use oftime.To independently or withthe Extended Scope Practitioner triage referrals and make onward referrals asappropriate.
This will involve the coordination of the patients complexpathway of care between healthcare professionals comprising the multidisciplinaryteam.
The multidisciplinary team consists of Allied Health Professionals,Healthcare Scientists, GPs, Doctors and Consultants in both primary andsecondary care and may involve other agencies social care.
Thus theclinician will be expected to refer on beyond their scope of practice.
To undertake comprehensiveassessment of patients, including those with complex presentation, usinginvestigative and analytical skills and to formulate individualised managementand treatment plans, using clinical reasoning and utilising a wide range oftreatment skills and options to formulate a specialised programme of care.There may be conflicting information to make a judgement on the best course ofaction to be taken for the management of the patients condition.To develop and maintainskills required to practice at an advanced level and use an extended scope ofpractice beyond that of Physiotherapy (including referral for andinterpretation of imaging, pathology reports, delivery of injection therapy).To be professionally andlegally responsible and accountable for all aspects of your own work includingthe management of patients in your care.
To ensure high standards of clinicalcare for the patients under your management and support others to do likewise.
Provide advice and supportto staff, patients and carers in relation to the general management of theservice users condition whilst understanding the broader implications for theindividual, family unit and carers.To provide spontaneous andplanned specialist advice, teaching and instruction to patients, relatives,carers and other professions to promote understanding of the aims of clinicaltriage and Physiotherapy and to ensure a consistent approach to patient care.
To be proactive inencouraging the use of a range of self-management strategies to patients ortheir carer/relative and in doing so overcome barriers to understanding.
Thiswill frequently require negotiation with patients on the return to activitieswhere cognitive, behavioural, and motivational issues may be significantbarriers to change.To maintain accurate, comprehensive,and up to date treatment documentation in line with legal and servicerequirements, communicating assessments and treatment results to theappropriate disciplines in the form of reports and letters.To assess capacity, gainvalid informed consent and have the ability to work within the legal frameworkwith patients who lack capacity to consent to treatment.
Safeguard theconfidentiality of all information discussed regarding the medical and socialcircumstances of the patient and their family.Professional:To maintain own clinicalprofessional development and competence (CPD) by keeping abreast of any newtrends, research evidence and clinical guidelines and incorporate them asnecessary into your work.To develop, monitor andmaintain high standards of practice for Musculoskeletal Physiotherapy withinthe MSK Assessment and Triage and physiotherapy services.To make recommendationsabout and lead on the development of evidence-led clinical initiatives withinthe Musculoskeletal triage service.To have a role in thedevelopment of ECCHs contribution to the local pathways within the scope ofthe MSK Assessment and Triage andphysiotherapy services.To take a leading role inthe clinical development of Physiotherapy staff within ECCH and their teachingand supervision.To be an active member ofthe in-service training programme and provide clinical leadership within themusculoskeletal team by attending and delivering presentations and trainingsessions at staff meetings, tutorials, training sessions in-house and by attendingexternal courses, practicing reflective practice and performing peer reviews.To liaise with GPs andother health care professionals to improve communication and quality ofreferrals and to provide them with training when required.To participate in the staffappraisal scheme.
To take a lead role for themeasurement and evaluation of the MSK Assessment and Triage Team working with the MSK Physiotherapy team were appropriate regardingcurrent practice through the use of audit and outcome measures.
To makerecommendations for change and implement changes to practice as necessary.Demonstrate a soundunderstanding of Clinical Governance and Risk Management and apply to the worksituation.Organisational:To be responsible forequipment used in carrying out duties and to adhere to any relevant policiesincluding competence to use equipment and ensure safe use of equipment byothers through teaching, training and supervision of practice.To be aware of Health andSafety and Infection Control aspects of your work and implement any policies,which may be required to improve the safety of your work area, including promptand appropriate recording and reporting of accidents to and ensuring thatequipment use is safe.Manage personnel (HR)issues in line with ECCH policies sickness and performance and takeappropriate action when identified as necessary for staff within MSK servicesin conjunction with the clinical coordinator for MSK services.To be responsible for thecollection of appropriate data and statistics for use in the MSKAssessment and Triage Team and MSK service and toprovide reports as necessary to the Clinical Coordinator for MSK services.To comply with theOrganisational and service policies and procedures and to be involved in thereviewing and updating as appropriate.To ensure that other MSKAssessment and Triage and Physiotherapy staffcomply with organisational and service policies when carrying out their work.
To act as a representativeof and an ambassador for the Company to ensure it is held in high regard.
To deputise for theExtended Scope Practitioner for the MSK Assessment and Triage Team.
