We are looking for registered pharmacists from all backgrounds (Hospital, PCN, GP, Community amongst others)to join our team in a variety of clinical areas.

o To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance.
o To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG).
To be responsible for the delivery of an agreed standard of pharmaceutical care to identified patients in order to achieve the safe and cost effective use of medicine.

Post holder will be expected to contribute towards evening, on-call, bank holiday and weekend rotas.

About the University Hospitals of Leicester NHS Trust: Job description Job responsibilities You will utilise your clinical skills and experience by providing clinical pharmacy services to wards and other areas, assisting with the training and development of others and supporting the improvement of the services.
You will work closely with the multidisciplinary team and will help maintain the high standard of services that are already provided.
Independent Prescribing is integral to our pharmacists roles.
In these posts you will have the opportunity to develop your clinical knowledge and specialist skills within one of the following Clinical Management Groups: Emergency and Specialist Medicine This team is based at the LRI and LGH and provides care to many area's including: Acute Medicine, Neurology, Stroke, Geriatrics, Diabetes, Endocrinology, Infectious Diseases, Rheumatology, Dermatology and Medical Day case.
Opportunity to work with many specialties to further develop and lead on local initiatives such as flow and improving discharge process Renal, Respiratory and Cardiovascular The pharmacy team based at the GH, provides care to patients with kidney problems including those requiring dialysis and kidney transplants.
It covers a large region including Leicestershire, Northamptonshire, Lincolnshire and Peterborough.
The cardiology team care for patients with cardiac conditions and those undergoing cardiac procedures.
The vascular ward cares for patients with vascular conditions both surgically and medically.
The respiratory wards care for patients with respiratory conditions, including specialist services in adult cystic fibrosis and tuberculosis.
Pharmacists also provide advice and support to patients receiving Pulmonary and cardiac Rehabilitation.
In addition, the team provide services to the Clinical Decisions unit (CDU), the admitting area for cardio respiratory patients.
Oncology & Haematology Oncology wards, Chemotherapy Suite, Haematology ward, Admissions unit, Day Care Unit and Bone Marrow Transplant Unit.
The team of Pharmacists work across the wards and areas administering chemotherapy.
They also provide the professional lead within the Aseptic Unit to oversee the aseptic production of cytotoxics and other aseptically prepared products.
Clinical trials are a core part of the work of the oncology & haematology team who work closely the trials team to set up and check clinical trials.
The team work closely with the palliative care team to ensure patients receive the best possible care during the terminal stages of their disease.
Person Specification Qualifications Essential Master's degree (or equivalent) in pharmacy.
Pharmacist registered with the General Pharmaceutical Council Post-graduate certificate in Clinical Pharmacy / Pharmacy practice or evidence of equivalent clinical experience Working towards completion of a Post graduate Diploma/MSc in Clinical Pharmacy/ Pharmacy practice Skills Essential Sound current clinical knowledge Evidence of communication skills, verbal/written.
Good interpersonal and communication Experience Essential Relevant and significant post-registration experience in pharmacy practice with relevant clinical experience Desirable Experience of working in hospital or equivalent sector
