Main duties of the job To assist in delivering a clinical medicines management service to patients.To provide expert advice on pharmaceutical matters relating to primary care, substance misuse and mental health services, this will include assistance with risk management and ensuring compliance with medicines legislation and local medicines management policies.To provide input into policy and procedures affecting the use of pharmaceuticals at both local and trust wide levels under the direction of the lead pharmacist.To liaise with dispensing pharmacy staff to support an effective dispensing and supply service.
To participate in clinical audits and research associated with medicines and pharmacy practice.To deputise for the Lead Pharmacist in their absence.

Job description Job responsibilities Ensure that all prescriptions and Medication Administration Records are clinically appropriate and meet all necessary legal requirementsHelp in the management of the repeat dispensing services ensuring full utilisation of this service.Run regular Medication review clinics for those patients on the repeat dispensing service and, if appropriate, use prescribing skills to add and maintain patients onto this system.Ensure medicines reconciliation is completed for all patients on arrival into reception.Ensure safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients.
This includes regular reviews of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects (Medicines Act, Mental Health Act) of medicines supply and compliance with the Trust FormularyAssist on the implementation of medicine related NICE publications, ensuring that recommendations form CNWL NHS Foundation Trust Medicines Management Group are considered and that systems for auditing and reporting on uptake are established.Be responsible for providing clinical pharmacy input to the multidisciplinary team caring for patientsPromote cost effective use of Medicines and other equipment.Promote the use of a preferred prescribing List (Formulary), liaising with appropriate healthcare professionals to ensure the cost effective use of clinically effective prescribed drugs.Ensure subordinate pharmacy staff providing a service to all prisons within the service line have adequate competencies to deliver such service.Be responsible for providing a clinical pharmacy service to groups of patients: Review prescriptions for accuracy and legality and identify actual or potential problemsEnsure all medications are recorded in the electronic prescribing system (SystmOne)In conjunction with the dispensing pharmacy, ensure the effective supply of medicines suitable for use (IP/NIP) and appropriately labelled for discharge or court if applicable.Ensure the effective outcomes of treatment with medicinesMonitor patients for potential and actual adverse effect of their medicinesEnsure all pharmacy interventions are recordedTake steps to ensure patients understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicinesSupport the planning of patients release with respect to medication, including the implementation of a dispensing for discharge system.Demonstrate professional accountability to patients Provide advice to clinicians on unlicensed medicines use and areas of practice where the evidence base is lackingWork across traditional boundaries as part of a fully integrated multidisciplinary teamActively participate in multidisciplinary committees as delegated by the Lead Pharmacist both within the organisation and outside to provide clinical pharmaceutical advice to support the development of disease management guidelines relevant to prison population and expert advice on practicalities of delivery clinical services within the prison setting.Assist the lead pharmacist in the development of Direct Patient focussed Clinical Pharmacy Services ie Pharmacy-Led Clinic services ie: Independent Prescribing, Medication issues directly to patient using PGDs, Patient Medication Reviews.Manage and make appropriate referrals to other members of the multidisciplinary team and within the pharmacy departmentBe able to critically appraise the literature and give accurate interpretation in order to improve use of medicinesReduce the risk associated with medicines use by contributing to the safe medication practice agenda.
This will include identification and reporting of medicines related errors on Datix.Develop and demonstrate expert clinical knowledge and motivate and inspire othersAct as a clinical role model and demonstrate the ability to provide safe, clinically effective and cost efficient use of medicines for junior pharmacy staffProvide pharmaceutical advice to health and associated staff as required.Advice provided is accurate, evaluated, impartial and up-to-date to ensure the safe, clinically effective and cost efficient use of medicines in patients within a prison setting.
Person Specification EDUCATION AND QUALIFICATIONS Essential Vocational Masters Degree in Pharmacy Experience of pre-registration training Substantive experience of post registration training relevant to hospital /prison pharmacy Member of the General Pharmaceutical Council of Great Britain Successful completion of short courses to the equivalent of a post graduate certificate in clinical pharmacy Desirable Working towards higher Degree/Diploma in Clinical Pharmacy Experience in clinical pharmacy in a prison setting Independant prescribing qualification Member of the General Pharmaceutical Council of Great Britain PREVIOUS EXPERIENCE Essential Demonstrate the ability to appropriately recommend, substantiate and communicate medicine related information to patients, carers and clinical staff Previous evaluated experience of mentorship and training skills To have undertaken clinical audit Training and education Desirable Previous experience in providing clinical prison pharmacy services.
