Job summary THIS POST IS FIXED TERM/SECONDMENT FOR 9 MONTHS DUE TO MATERNITY COVER.

This is an opportunity to join our East Home First team, covering band 6 maternity leave for 9 months.
The post will involve working closely with inpatient therapy and nursing teams, community resource teams, and district nursing colleagues to continue to build on good MDT working.
The post holder will have experience and personal interest in community rehabilitation and home first approaches.
The caseload will be varied with a range of complex needs.
Main duties of the job To provide holistic OT assessment, goal setting, individualised treatment and rehabilitation programmes, care planning and delivery.Regular review and discharge of patients.
To manage a varied caseload, seeking support from senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice.
To be actively involved in, and lead on, projects towards the development & delivery of the service and to contribute to new developments and initiatives.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
