Job summary We have a exciting opportunity for a Band 6 Occupational Therapist working hours per week to help support ,provision of our patients in the Community .
Our team is based in The Fountains Health centre.
You will be part of an Integrated Care Community Team and be responsible for the Therapy provision for our patients.
This role is working in the Chester East Care Community Team, although the successful candidate will need to be flexible to support our colleagues within our 8 Care Community Teams in the Neighbourhood Group.
**We offer a relocation package of up to £8,000** The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

Main duties of the job To work autonomously as a specialist therapy practitioner within an integrated Care Community Team to deliver a high-quality Occupational Therapy service through assessment and treatment of patients referred with complex rehabilitation needs To work as a member of a multi-disciplinary team and communicate information to those agencies (either health or social background) involved, to meet the needs of the patient & plan effective care.
To participate in departmental In-service Training and Journal Club activities to further consolidate knowledge base and specialist clinical skills.
Participate in the CPD programme to share these skills with therapy and nursing colleagues.
To provide supervision, teaching and support to senior and junior staff, assistant staff and act as a clinical educator for undergraduate students.

Person Specification Qualifications Essential Diploma/Degree in Occupational Therapy Evidence of relevant post-graduate courses.
Fieldwork educator's qualification HCPC registration Desirable Member of Royal College of Occupational Therapy (RCOT).
Knowledge and Expertise Essential Knowledge of Occupational Therapy specific theoretical frames of reference in line with evidence-based practice Professional guidelines and codes of conduct Critical appraisal Trust and Services policies procedures Evidence of CPD and portfolio demonstrating interest in a specialist area Commitment to Continuous Professional Development Access to transport for work purposes.
Adherence to the Code of Ethics and Professional Standards of the HCPC Experience Essential Minimum of 2 years post graduate experience, including a broad range of working in relevant fields Commitment to training and development Team working Reflective practice Desirable Experience of student clinical supervision Experience of day-to-day supervision of Band 5 and/or support staff
