This post is offered as an advanced specialist pharmacist post but is also appropriate for candidates with less experience as a specialist to advanced specialist development/ progression post.If appointed as a specialist pharmacist the post holders will be expected to develop a portfolio of evidence to support their progression in this post.Applications are welcome from pharmacists with relevant hospital experience - experience in antimicrobial pharmacy is not essential.
Main duties of the job Successful candidates will be joining a team of pharmacists providing and developing the antimicrobial stewardship programme at our Trust.
The post holder will be involved with many aspects of our antimicrobial stewardship programme including guideline development, clinical audit, provision of web-based resources and training/education.
They will apply their clinical skills in reviewing patients and will have an opportunity to attend multidisciplinary team meetings.As a clinical role model and antimicrobial champion, you will need to consistently demonstrate the ability to provide safe, clinically effective and cost-efficient use of antimicrobials in your clinical role.Developing your prescribing practice in antimicrobials is encouraged; if you are not already a prescriber support will be offered to develop this skill in the future.We offer the equivalent of 1 day per month protected development time for pharmacy staff (pro rata for part time staff).The role is predominantly based at the Leicester Royal Infirmary; travel to other sites is to be expected, as is participation in core pharmacy services (weekends, bank holidays and on-call).

About the University Hospitals of Leicester NHS Trust: Job description Job responsibilities To support the development of optimum pharmaceutical care to patients within theTrust and wider Health Community as partly of a multidisciplinary team within thearea of speciality.
To lead, develop and deliver training for pre-registration and qualified pharmacistsand medicines management technicians within the area of speciality.
To line manage other pharmacy staff as required.
To lead the education and training to all other health-care staff in accordance withnational and local recommendations.
To deliver a highly specialised level of clinical input using advanced theoretical andpractical knowledge in area of Specialty, also demonstrating high level of knowledgein other areas so as to take relevant co-morbidities into account when makingtreatment decisions commensurate with the grade, including attendance on wardrounds.
To demonstrate professional autonomy, involving highly complex facts or situations, which require analysis and interpretation of data, leading to the implementation of a treatment and management strategy.
To initiate, organise and manage initiatives to promote prudent and cost effectiveprescribing within the speciality.
To report on financial expenditure within Speciality, suggesting areas in which cost savings may be made and evaluating changes in expenditure resulting from policy changes.
To provide guidance and input into CMG prescribing action plans and monitorprogress where relevant to the speciality, adjusting the plans as necessary.
To provide expert advice and support the submission of new medicine requests to the UHL Therapeutic Advisory Service (TAS), as necessary and to advise and support initiatives relating to Specialty for the Leicestershire Medicines Strategy Group.
To provide specialist advice to patients, medical and other clinical staff on any drugelement of specialist guidelines and to multidisciplinary care teams, to optimisequality of patient care.
To develop shared guidelines / care agreements for medicines within the area ofspeciality to reduce risks associated with transition between secondary and primarycare.
To provide highly specialised medication related advice.
This will includeindividualisedadvice on medication dosing, drug choice, administration, drug level monitoring anddose adjustment, the use of unlicensed medicines, the use of medicines outside thetermsof their product licence and advice on the safety and storage of medications.
To support medicines optimisation within the area of Speciality, to include review ofmedicines-related errors and formulation of strategies to reduce these errorsthroughout the Trust, developing strategies to promote safe medicines use andsupporting patient education.
To work with the wider Health Community and multidisciplinary team to develop and improve medicines-related aspects of patient care pathways to reduce admission and readmission to hospital in area of speciality.
Collate data and produce reports on drug use within the specialty and prescribing audits as required by the service.
Assist in the production of Patient Group Directions (PGDs), with appropriatemedical/nursing staff.
Ensure PGDs are regularly reviewed, and provide PGD trainingfor CMGs as required.
Actively pursue audit, research and development within the area of speciality and thePharmacy Department.
Publicise the results of such work through presentations andpublications.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
To undertake other such duties as required commensurate with the post includingdispensary duties, evening and weekend working and on call-when required.
Person Specification Training and Qualifications Essential Pharmacist registered with the General Pharmaceutical Council Postgraduate certificate in Clinical Pharmacy/Pharmacy Practice or evidence of equivalent clincial experience Working towards completion of (or have completed) a postgraduate diploma/MSc in Clinical Pharmacy/Pharmacy Practice Desirable Membership of a professional body MRPharmS or UKCPA Registered as an independent prescriber Evidence of leadership training and skills, particularly change management.
Experience Essential Approaching 2 years (minimum) post registration experience in pharmacy practive with relevent clinical experience Desirable Experience of teaching/tutoring Experience working in antimicrobials/infection management Evidence of teaching or training healthcare professionals Evidence of involvment in service development Experience of leading or working within a multidisciplinary team Experience of supervision/management of staff Skills Essential Evidence of verbal presentations made to professional groups
