Job overview This is a brand new, exciting opportunity for an Occupational Therapist or Physiotherapist to work within a Stoma specialist nursing team to support patients before and after stoma surgery.
There is only one other AHP Stoma post in England currently, which has been a huge success and we are committed to improve our patient experience and outcomes by enhancing our existing excellent nursing service with an AHP colleague.
The post holder will provide a comprehensive therapy service and be a key team member in the delivery of stoma care services to enable and support timely and effective care, appropriate assessment and intervention for patients with stoma care needs in hospital, their own homes and clinics.
They will, as part of the specialist stoma team, support the undertaking of ensuring patients and carers receive high quality care, support and advice.
The post holder will be a registered Occupational Therapist or Physiotherapist who is already a Band 6 or is ready to develop into a Band 6 role.
Main duties of the job The specialist AHP will work in the Stoma service to assess, manage and provide treatment to a caseload of service users with complex and/or multiple needs who are on the Stoma nurses caseload and are awaiting stoma surgery or have recently had a new stoma.
They will be working closely with the MDT and provide clinical supervision to the community therapy staff and assistants as required.
They will be able to work in all five care communities of Nantwich, SMASH, Northwich, Crewe and Winsford within patients' homes, residential and nursing homes and within the inpatient setting at Leighton hospital.
To provide the specialist practitioner function in the delivery of specialist care to patients who have been referred to the Stoma Specialist Nursing Service.
The post holder will also be involved in developing and delivering high quality, specialist patient centred care and working with the associated clinical team to ensure this is incorporated within the Business Group and Trust objectives.
To deliver training both formally and informally within the specialist field of Stoma Care.To act as a role model ensuring practice is the most up to date and delivered in a professional manner at all times.
It is recognised that this is a unique and brand-new role with currently only one other stoma AHP in the country, therefore a competency package will be devised to upskill the postholder with the necessary knowledge required for the specialist role.

Detailed job description and main responsibilities ·Manage a clinical caseload with autonomy, work without direct supervision·Undertake comprehensive AHP assessment, provide a therapeutic diagnosis (using investigative and advanced analytical skills, specialist listening, physical and observational skills, interpret clinical findings and documentation) and implement agreed treatment plans if appropriate, including the treatment of complex service users with multiple pathologies in a manner that respects people's privacy dignity and individuality.·Provide individual or group therapy as appropriate to setting.·Implement monitoring and evaluation of treatment in order to measure progress and outcomes to ensure effectiveness of interventions and service delivery.·Develop programmes of care to support discharge from service.·Be responsible for equipment used in carrying out clinical duties, and to adhere to policy, including competence to use equipment and to ensure safe use of equipment by others through teaching, training and supervision of practice.·Record, maintain and store accurate, comprehensive and up to date documentation, in line with legal and local requirements.
Communicate outcomes of assessments/intervention to appropriate disciplines in form of letters/reports/discharge summaries·Arrange appointments (as applicable), work closely with MDT, attend case reviews/case conferences/variety of MDT internal and external meetings/forums·Contribute to the safeguarding of our Service users/families as appropriate.·Assess capacity, gain valid informed consent and have the ability to work within a legal framework with service users including those service users who lack capacity to consent to treatment, within your organisation's policies.·Be professional and legally responsible and accountable for all aspects of your own work including the management of Service users under your care.·Maintain professional registration with HCPC.·Work within organisation, HCPC and CSP/RCOT guidelines.·Have a working knowledge of national and local standards and monitor own and others quality of practise against these.·Work as part of a team to ensure effective communication and delivery of care.·Provide and receive, with care and integrity, sensitive information about service users from the service user, carer and other professionals and to manage sensitive or contentious information appropriately.
