As a social worker within hospital discharge you will: Be person centred and reinforce a patient's choice and control.
Promote a client's wellbeing throughout their journey in the acute, IMC and community settings.
Complete Care Act 2014 compliant assessment, support plans and reviews.
Complete Mental Capacity Assessment and best interest processes for clients unable to make specific decision.
Knowledge of the Deprivation of Liberty Safeguards.
Experience of Safeguarding processes and procedures.
Source provisions both formal and informal in support of client's needs.
Have experience of MDT working.
Have knowledge of related legislation, policy, and frameworks.
Be able to work as part of a duty team on a rota.
