Social Worker

o complete the ongoing assessments for young people at crisis on the ward, or routine and in the community.
* To complete all assessment paperwork, contact any agencies and make appropriate referrals.
* To complete risk assessments as appropriate and safety and care plans * To work with other partner agencies eg children's services / safeguarding etc.
* To look at undertaking ongoing support work for young people in crisis, to refer as appropriate and then review on return to services.
* To complete sessional work with the young people on an ongoing basis as work directs.
* To work with risk and complexity and priorities * To manage a caseload and escalate any matters of concern or uncertainty.
* To work in a team and support and be supported by the team.
Full time Monday to Friday 9-5, Can have some flexibility in hours, and may be asked to work an occasional additional day TPBN1_UKTJ
