The Exeter (West) Integrated Health and Social Care team is looking to appoint a part time (30 hours per week) Social Worker for a fixed term period of 12 months.The team is made up of a range of health and social care professionals including therapists, nursing staff, community matrons and social care professionals.
We also work alongside and in partnership with our GP services.

This includes investigating safeguarding concerns, conducting statutory assessments, court work, commissioning support, and developing support plans.
You will collaborate with partner agencies, carers, and families to build networks of support that can help people maintain their independence.

You will receive regular supervision.Hybrid working arrangements are applicable to the role, this includes elements of office-based working, client visits and working from home.Social Worker Career Pathway We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels.

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
