As such you will work alongside Social Care Assessors, Occupational Therapists, Occupational Therapy Assessors to ensure the most appropriate route is accessed for residents to receive appropriate care and support considering the most up to date services, equipment, technology and techniques are being utilised.
You will give information, advice and guidance whilst helping to promote wellbeing and independence and preventing, reducing, or delaying the need for services.
When necessary, you will ensure that the people using our services move seamlessly through to other teams whilst recording information using the IT systems in place.
Therefore, the ideal candidate will require skills such as having a professional manner, be a skilled communicator, be organised and have prior good computer literacy.
You will receive training on specific IT systems used.You will require a thirst to learn about the up-to-date available services in the community such as voluntary agencies and charitable services.
