As a Social Prescribing Link Worker you will be employed by Wolverhampton Voluntary and Community Action (WVCA) and attached to one of 6 Primary Care Networks in the City.
Are you the right person for the job?
Educated to Degree level, NVQ Level 3, or equivalent level of experience Proven experience of working within a client-centred context Experience of working in the Voluntary and Community or Health sectors, with knowledge of both Experience or knowledge of the health and social care environment Experience of working across multi-agency settings and across teams Experience of collaborative working Excellent communication and networking skills Knowledge and understanding of community development values and principles Knowledge of health and social care issues Organised and digitally literate An understanding of safeguarding and related issues Experience of managing risk, including when to involve medical staff Experience of working within diverse communities and an understanding of and commitment to equality of opportunity and good working relationships Able to travel within Wolverhampton on a daily basis including visiting people in their own homes While not a requirement of the post, an ability to communicate in alternative languages would be an advantage What will your role look like?
Accept referrals from GPs and other agencies Provide personalised, holistic support Build trust with vulnerable individuals Co-create simple care plans based on individual priorities and motivations Use tools like PAM to assess and monitor health improvements Assist eligible individuals in exploring personal health budgets Connect people to local voluntary organizations and community groups Participate in health coaching as needed (training provided) Meet customers at convenient locations, including GP surgeries and homes Strengthen and expand local Voluntary and Community sector services Identify local service needs and gaps, and collaborate on solutions Promote holistic service provision to aid in healthcare transformation and prevention.
Identify and replicate successful local initiatives Develop understanding of local assets (skills, knowledge, activities, services) Build relationships with key staff in GP practices and other health and social care professionals Play a key role in Multi-Disciplinary Teams Collaborate with health professionals to address practice issues and enhance service provision Raise awareness of Social Prescribing across the City Participate in regular supervision and training as required Record monitoring information using various systems Perform tasks consistent with the role's level and scope, ensuring timely and effective work delivery Attend team meetings and partnership activities as needed Train as a health coach Produce case studies and contribute to reports Engage in other role-related activities in a flexible and developing environment What can you expect in return?
