It has been identified as a key component of NHS England's Universal Personalised Care objective contained within their Long Term Plan, as well as London's Health Inequalities Strategy for healthy communities published by The Mayor's Office in 2018.
Both strategies recognise the influence of social, emotional, economic and cultural factors on health and well-being and the importance of community connections to improving health and wellbeing, self-care and self-management of long term conditions.
Family Action is proud to be the provider of the existing local Social Prescribing Service commissioned by City & Hackney CCG delivering since 2014, which recently won Best Larger Project at the inaugural Social Prescribing Awards 2019.
We are now delighted to be expanding the provision through additional funding provided by City & Hackney Primary Care Networks/Neighbourhoods.
Main duties of the jobYou will work as part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing 1:1 personalised support to children and young people, individuals, and families utilising a holistic and strength-based approach, to help identify the wider issues that impact on their health and wellbeing and work together to co-produce a wellbeing plan to address these needs.
You will have excellent holistic assessment skills, as well as interpersonal and communication skills that enable you to build trusting relationships which support individuals feel empowered with their wellbeing.
You will forge strong links with local VCSE organisations, community and neighbourhood networks, building on what is already available, to create a menu of community groups and assets to connect individuals to as part of their wellbeing plan.
You must be a good listener and have experience of working positively with individuals facing complex social and emotional challenges that impact on their wellbeing.
Being creative as well as having a solution-focused attitude to removing barriers to health and wellbeing is essential.
You will also have strong leadership qualities, experience of supervising staff and report writing to support strategic and organisational requirements.
Job responsibilitiesTo undertake whole family/children/young people assessments/holistic assessments and co-design Health and Well-being Plans with service users, identifying support needs to ensure maximum engagement in improving health and well-being.To provide service users with continuity and a co-ordinated experience of care, remaining point of contact throughout the individual's social prescription.Work with CYP and families to co-produce a simple personalised support plan to address their health and wellbeing needs based on their priorities, interests, values and motivations.Be proactive in encouraging self-referrals and connecting with all local communities, removing barriers particularly for those communities who may be under-represented in this service.Where appropriate, carry out home visits and introduce CYP/families to community groups, activities and statutory services.Forge strong links and collaborative relationships with local VCSE organisations, community and neighbourhood level groups.As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN, attending relevant MDT meetings.To work in partnership with all voluntary and community organisations to build a comprehensive database of local resources.Provide high quality supervision to staff, volunteers, and students, and monitor the safeguarding responsibilities and training of staff.Act as safeguarding lead for the CYP and Families Social Prescribing service and work according to Family Actions and City and Hackney's safeguarding policies and procedures.To carry out case audits on a regular basis, complete audit reports and share information with the SP worker/s and the Hackney services manager.Set up and maintain comprehensive data and evaluation systems, including individual health outcome tools.Attend the steering group including representation from all stakeholder groups.Provide quarterly comprehensive outcome-focused reports detailing the progress of the service.To recruit, train and supervise volunteers, matching them to individual service users.To keep records of your work and adhere to confidentiality, information sharing protocols.To implement the principles of Family Action and Hackney's Equal Opportunities Policy.To work flexibly as required by the service.To comply with Family Action's Health and Safety Policy, Data Protection Policy.To ensure you have an understanding of, and comply with Family Action's procedures for promoting and safeguarding the welfare of children and vulnerable adults.Person SpecificationExperienceExcellent children, young people and families focused, holistic assessments skills.Experience of training, supervising, motivating, and developing volunteers and staff.Proven skills in collating information and data on community resources.Excellent record keeping skills and the proven ability to write comprehensive reports.Excellent IT skills and ability to do own administration.A confident and professional approach to working with a variety of stakeholders.Excellent written, verbal, listening and presentation skills.A proven understanding of safeguarding for children and vulnerable adults.The ability to work autonomously and to plan, prioritise work under pressure.A commitment to equal opportunities and an understanding of the impact on individuals, families and communities of EDI related issues.Ability to work hours in a flexible way, including occasional evenings and weekends.QualificationsProfessional Health/Social Care/Mental health qualification to at least Degree level.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
£36,138 to £39,627 a year Salary is inclusive of the Inner London Weighting.
