Job summary Social Prescriber Link Workers play an important role within a Practiceto proactively identify and work with people, to provide non-medical supportthat improves their health and wellbeing.
The service will support patients toaccess all available opportunities by linking individuals with community andvolunteering support so they can take charge of their own health andwell-being.The role is an integral part of the Practicemulti-disciplinary team and should address the wider determinants of health,such as debt, poor housing and physical inactivity.
You will be required to providepersonalised support to individuals, their families and carers to achievebetter health outcomes.
Main duties of the job You will be required to assess, plan, develop, implement and evaluate programmes & individual care plans that promote health, primarily by services & opportunities that are available in the community.
You will be required to expand the referral gateway to take referrals from a wide range of agencies in line with Practice requirements.
To promote social prescribing, its role in self-management & the wider determinants of health.
Assess and integrate issues surrounding what the patient sees as important for them into the overall therapy process.
Be proactive in developing strong links with all local agencies in line with the social prescribing implementation plan to encourage referrals recognising what they need to be confident in the service to make appropriate referrals.
Produce a simple personalised care plan to address the patients health & wellbeing needs by introducing or reconnecting people to community groups or statutory services.
You will be required to liaise with patients, their families/representatives, healthcare professionals, social services, GPs & other organisations to ensure that the care needs of the patient is being met & the desired outcomes achieved.
A high level of communication is required to work with individuals to establish their goals.Coaching skills and motivational interviewing will be used to motivate, encourage & support individuals.Difficult conversations will be required to understand why individuals decline to work to the agreed programme.

Job description Job responsibilities You will give advice to and help individualsestablish a programme that will address their needs, which could be around abroad range of health and social issues such as social isolation, mentalhealth, long term conditions etc.You will be responsible for assisting the practicein developing a holistic health and social care service which meets the needsof the practice population.
This may include proposing changes to currentworking practices and procedures and evaluating the effectiveness of these.
You will provide telephone or home visits when theneed arises for patients unable to be seen at the practiceEffectively prioritise tasks to meet unexpectedemergencies and changes in care packages Develop trusting relationships by giving peopletime and focus on what matters to themYou will need to manage and prioritise your owncaseload in accordance with the health and wellbeing needs of the population You will be the lead for organising and chairingthe MDT, Safeguarding and PPG meetings as necessaryYou will act as the clinical lead at the PPGmeetings forging strong links between the PPG members and the PracticeAttendance and contribution at Practice Meetings,MDT Meetings, Clinical Meetings and Safeguarding Meetings and any othermeetings as necessary.Informand advise practice staff about what services are available within thecommunity and how and when patients can access them.
Workcollaboratively with all staff to contribute towards supporting localorganisations and community groups to become more sustainable through sharinginformation regarding any gaps or problems identified in local provision withcommissioners and local authoritiesThe post holder will be responsible in planningtheir day to day work but will also be responsible for planning and organisingactivities or programmes some of which may be ongoing.Establishstrong links with Practice staff and contribute to the wider aims andobjectives of the organisation Be achampion to promote co-ordinated care and support for allLiaisewith a range of multi-disciplinary professionals who are involved in apatients care, ensuring a smooth and coordinated approach, especially wheremultiple agencies are involved.
Supportthe Practice vision in the management of care and avoid unnecessary hospitaladmissions and unnecessary GP referrals.
Youwill develop a more robust and effective way of working in managing registersfor patients who are on the MDT, Safeguarding, Palliative and Carers register.
Ensuring that we are achieving better healthoutcomes for our more vulnerable patients.

Desirable Training in motivational coaching and interviewing or equivalent experience Evidence of continuous professional development with regard to wellbeing.
Mental health awareness training Personalised care institute training Experience Essential Experience in coaching, counselling, or other appropriate background relevant to the role Proven experience of planning and goal setting to help improve health and well-being outcomes Has experience in relationship building skills across a variety of different networks Knowledge of Primary Care and the voluntary and community sector Experience of delivering general and/or specialist advice and information to clients An understanding of Social Prescribing and how it may contribute to health and wellbeing Good IT Skills and the ability to complete all necessary patient records A confident and professional approach to working with a variety of stakeholders A proven understanding of safeguarding and ability to implement relevant policies and procedures The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working Desirable Use of SystmOne Experience of working in a GP Practice
