TheSocial Prescribing Practitioner will co-productively draw up a personalisedplan supporting patients to access and engage with the extensive range ofsupport available in the community.Your role would involve workingon a one-to-one basis, offering person centred appointments/care to support patientsneeds, connecting patients to community groups and statutory services forpractical and emotional support when appropriate.

Social Prescribing is ashort-term intervention service working towards building a diverse andsustainable network of support for those using the service.The social prescribing approachempowers individuals to take control of their own health and wellbeing.
Yourrole would involve working on a one-to-one basis, offering person centredappointments/care to support patients needs, connecting patients to communitygroups and statutory services for practical and emotional support whenappropriate.
Your roleis central in developing a relationship with patients, you will resourceful andcreative to find the most appropriate support and build patient relationships.You should have experience of working positively with people facing social andemotional challenges.
You will beworking with people from diverse cultural and social backgrounds and will needto work confidently and effectively in a diverse and challenging environment.
You will be required to undertakehome visits so you will be required to hold a full clean driving licence andaccess to your own vehicle with insurance coverage for business use.Were looking for someone who has: Experience of working with people on a one-to one basis in an information,advice and guidance health care sector The ability to plan, develop and manage your own caseload to ensure patientobjectives are met To provide person-centredsupport, agreeing well-being action plans with individual participants,identifying and planning support needs to ensure maximum engagement and improvehealth, wellbeing and confidence Knowledge confidentiality and information sharing protocols Knowledge and practical experience of local safeguarding processes and anability to operate within relevant policies and procedures to safeguard ourPatients Youll need to be: Well organised and resourceful with abilityto work on own initiative, meet deadlines and monitor own performance Flexible within the practice to attend anymeetings in relation to your role, MDTs in relation to patients on yourcaseload.
You will be required to workover four sites within the Immingham, Keelby and Grimsby area (Meridian PrimaryCare Network)You will have the ability to work as part of a team, some lone working may berequired (subject to risk assessment).You will be confident, outgoing and have excellent communication skills.Able to record accurate information on IT systems.
You will also Be able to think outside of thebox, be adaptable in ways of working, continuously improving your skills tosupport our patients effectively.Essential Health and social care, coaching,counselling, or other appropriate background relevant to the role Proven experience of planning and goalsetting to help improve health and well-being outcomes Proven track record in relationship buildingskills across a variety of different networks Knowledge of Primary Care and the voluntaryand community sector Experience of deliveringgeneral and/or specialist advice and information to clients An understanding of Social Prescribing andhow it may contribute to health and wellbeing Good IT Skills and the ability to completeall necessary client records A confident and professional approach toworking with a variety of stakeholders A proven understanding of safeguarding andability to implement relevant policies and procedures The ability to workautonomously and to plan, prioritise work under pressure and adapt to newmodels of working A commitment to equal opportunities A commitment to Continuous ProfessionalDevelopment and undertaking relevant training opportunities when they becomeavailable.Communication10.

Promote social prescribing,its role in self-management, addressing health inequalities and the widerdeterminants of health16.
