DBS Check: Enhanced Closing Date: 15/12/2024 at 23:00 Reference: 710237 710237 We have an exciting opportunity available for a Social Care Practitioner to join our proactive and award-winning Adult Social Care team here at Wokingham Borough Council, on a part-time (26 hours per week), permanent basis.
As a Social Care Practitioner, you will use a strength-based approach to maximise individual's abilities to live as independently as possible and remain safe in their community.
You will work under the supervision of a registered practitioner to undertake direct work with individuals and their carers.
Adult social care provides a range of interventions to adults who need extra support to stay healthy, play a part in their community and lead as fulfilling a life as possible.
The service provides personalised, practical support to help people whose needs arise from illness, disability, old age and social disadvantage.
This is an excellent opportunity to develop and advance to a Grade 6 once all progression criteria's have been met.

Key responsibilities: To work under the supervision of a registered practitioner to undertake direct work with individuals and their carers.
Support carers to continue in their caring role for as long as they are willing and able to do so.
Seek to prevent, reduce, and delay the need for formal care and support using a strength-based approach which includes provision of tailored advice, information, and a range of interventions.
Assess customers and their carers, contribute to setting of personal budgets, create and implement support plans and review these as appropriate.
Comply with statutory guidance and within legislative frameworks for example Care Act 2014 , Mental Capacity Act 2005 etc.
Candidate requirements: GCSE Math's and English or equivalent Working knowledge of key legislation and guidance that relates to the provision of adult social care.
Awareness of the principles of safeguarding Experience of working with vulnerable adults Competent level of IT skills and ability to learn and use adult social care recording system Our Area Wokingham Borough is set in the heart of Berkshire with excellent transport links to Reading and central London.
