Welcoming applicants seeking between 14 - 37 hours per weekLocation: Royal Devon and Exeter Hospital (Wonford)An exciting opportunity has arisen in the Hospital Discharge Team where you can be at the forefront of supporting people within both the hospital and community care settings.
You will be responsible for facilitating discharges from Wonford Hospital and assessing the service users care needs once they have been discharged.
We have adopted the Discharge to Assess Model which is facilitating complex discharges from the hospital.
You will be based at Royal Devon and Exeter hospital, meeting patients to support their discharge from hospital as well as assessing service users post-discharge to formulate long-term care arrangements.
You will be working collaboratively with our hospital and community-based colleagues and within a whole integrated system.The role would provide an excellent opportunity for anyone who is looking to progress their social care career.
Experience of working within adult social care settings would be advantageous.
You will be working alongside a health and social care team which comprises Senior Social Workers, Registered Nurses, Occupational Therapists, Physiotherapists and other Social Care Assessors.This role would suit an individual looking for a fast paced and varied role that will include the completion of strengths based Care Act assessments, mental capacity assessments, co-ordination of care, placements and ongoing social care support from and following discharge from hospital.
You will also have the opportunity to support and advocate for service users on end of life and Continuing Healthcare pathways.The nature of this work requires you to be highly organised with strong recording skills and in turn you will receive excellent support in a busy and friendly team.
You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under complex and challenging situations.
You need to be IT literate and willing to develop your skills.

Applicants must hold a valid UK driving licence, have access to a car, and be able to cover a large geographical area (exceptions may apply under the Equality Act 2010).
You will be required to be available for Saturday working (approximately 1 Saturday required every 5 weeks).
You will receive a day of in lieu for Saturday working and an enhanced rate of pay.You will have access to a range of training opportunities and be by a friendly, welcoming team.Interview Date: 06/02/2025 – 07/02/2025
