Job summary Closing Date: (this may change dependent on response) Shortlisting to take place after closing date: commencing Interview expected to take place in the week following shortlisting: commencing An exciting opportunity for a qualified and highly motivated registered nurse to join the expanding Elderly virtual ward team to deliver frailty in reach into A+E and to support the development of a front door frailty same day assessment unit.
The successful post holder will work as a dynamic and proactive specialist nurse within our very successful and developing Integrated Frailty service.
The service operates over 7 days and provides the frail population of Bradford the benefit of a holistic Comprehensive Geriatric Assessment.
You will be based at the front end of the hospital, working collaboratively to start the assessment process from the moment the patient arrives in the emergency department or our assessment wards, including the frailty same day emergency care unit.
As a team member you will be part of a friendly, supportive, and professional integrated team which comprises of Consultants , Geriatricians, Nurses, Occupational Therapists, Physiotherapist, Coordinators, Advanced Clinical Practitioners, Pharmacists, Social services and Community services.

Person Specification Experience Essential Significant Community / Ward based experience of Frail older patients with complex needs.
Experience of teaching / assessing / supervising others in a clinical setting.
Desirable Experience of using Systmone / IT systems.
Proven experience on managing a team, including management of rotas, managing competencies, resources and staff.
Skills Essential Good communication skills and ability to deal with sensitive information Demonstrates ability to influence and negotiate with others Able to assess plan, implement and evaluate programmes of care.
Able to work as an effective team member Desirable Able to manage difficult situations.
Basic computer skills including ability to use Microsoft Word and Electronic Patient Records Knowledge Essential Up to date knowledge of current clinical and professional issues Knowledge of evidence based practice.
