Job summary We have an exciting opportunity for a Registered Nurse tosupport thetransition of young people with Life Limiting or Life-Threatening Conditionswho access hospice care and are transitioning from Childrens to Adult SocialCare, Education and/or Health Services.This role will require you to travel across all Andy's catchment areas soa clean driving license and access to a vehicle is essential.
Flexibility isalso required to be able to undertake visits at a time convenient to the youngperson and their family.
Main duties of the job As part of this role, you will build relationships withadult partners, to support the transition of young adults with progressivelife-limiting conditions from Andys Childrens Services into appropriate adultpalliative care services.
You will also support the development of pathways andresources.You will lead on Transition between childrens and adultservices for the organisation, acting as a resource to colleagues regardingtransition issues and offer specialist advice as required.
This may includeoffering training and support to other professionals in supporting young peoplewith complex needs.As a Registered Nurse or AHP it is essential that you hold aregistration with the Nursing and MidwiferyCouncil or Allied Health Professional Qualification on the HCPC registerin addition, would be advantageous to possess post registration experience inworking with young adults.
