Job summary Hours Per Week.We are looking to appoint a motivated & forward-thinkingSenior Therapy Assistant to jointhe Neighbourhood Rehabilitation Service (NRS) which encompasses theIntermediate Care team and Long-Term Condition Pathway, working across BarnsleyCommunity.The teamprovides rehabilitation to patients in their own home & residential care.The service provides specialist community therapy assessment & treatment toimprove health & independence.
In addition, the service offersrehabilitation to enable earlier discharge from hospital or to prevent hospitaladmission.You willwork as part of amulti-disciplinary team, followinginitial assessment by a registered health care professional you will exerciseinitiative & judgement in care planning, goal setting, delivery,implementation & evaluation of care, working collaboratively & inpartnership with othersYouwill contribute to enabling service users to reach their potential & develop knowledge to promoteholistic care, including supporting patients who have longer termrehabilitation needs.Hours of work will include weekends and bankholidays.All employees of the Trust are strongly encouraged to befully vaccinated against COVID-19 to protect patients.

Job description Job responsibilities Undertake delegated duties under the supervision of registered health care professionals.Consulting as necessary and working collaboratively within the health care teamto deliver therapeutic interventions, psychological and physical care to adefined group of service users.Deliver evidence based planned care,which is safe and effective, in a manner which is caring , compassionate andenables service users to reach an optimum level of functioning, independence, rehabilitationand recovery.Provide care within well establishedpolicies, procedures, protocols, guidelines, and established clinical careplans, whilst acting within the required sphereof clinical competences for the roleat all times.Followinginitialassessment by aregistered health care professional, exerciseinitiative and judgement in, care planning, goal setting, delivery,implementation and evaluation of care.Workingcollaboratively and in partnership with others, develop knowledge to promoteholistic care.Demonstratesound knowledge of the careand treatment programmes of individual service users toenable the post holder to contribute and informthe decision making of registered health care professionals.Make appropriatecare adjustments, within the agreed level of autonomy for the post, andbe able to communicate anychanges to service users, carers and the health care team.
This may includeregular assessment, evaluation, reportingand escalating relevant information to appropriate health and social careprofessionals.Undertakeenhanced training and preparation to meet the specific clinical needs of theservice and to equip the post holder for an enhanced leadership role.Contribute to enabling service users toreach their potential, including the utilisation of public health approacheseg, providing advice and support or signposting to the appropriate service.Undertake and maintain the requiredstatutory, mandatory and core training as required by the specific area ofservice.
Person Specification Personal Attributes Essential Responsive attitude and approach Dress appropriately for the environment A Current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability) Qualifications Essential A-C in Maths/English GCSE or equivalent Level 5 Apprenticeship/Foundation degree in Health & Social Care or equivalent relevant to the post Or, level 3 Apprenticeship/Diploma in Health & Social Care with the ability to demonstrate competencies at level 5 and will willingness to work towards achievement of level 5 qualification.
Care certificate or required to achieve within 12 weeks of commencement (where relevant to the post) Desirable Experience of the speciality area Group Facilitation skills Willingness to undertake further study/development Special Knowledge and Skills Essential As appropriate to the role: Demonstrates excellent customer care skills Of care procedures, practice, recovery model, care pathway and person centred care Provide and receive practice supervision Enthusiastic, able to work flexibly on a 24hr shift pattern to meet the needs of service users (where appropriate to the role) Demonstrates safe decision making skills commensurate with role.
