The starting salary for this role is £46,399 per annum, working 36 hours per week.
Surrey Heartlands Health and Care Alliance is seeking a (Senior Social Worker) Community Health and Social Care Integration Specialist to join our mission in delivering neighbourhood-based, integrated care.
This newly created and innovative role will bridge the gap between urgent and proactive care for individuals on the frailty pathway, focusing on admission avoidance and providing holistic support at home.
