Job summary An exciting opportunity has arisen for a enthusiastic and motivated Senior Rehabilitation Assistant to join the Neuro Early Supported Discharge Team within Hampshire & IOW Healthcare NHS Foundation Trust.
A progressive Band 3-4 would also be considered depending on experience; this would be discussed at interview.The Neuro Early Supported Discharge Team is a Multidisciplinary team providing specialist intensive rehabilitation to clients with acute stroke and other neurological conditions on discharge from hospital or as community admission avoidance.
The post holder will be working alongside a team of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Nursing, Clinical Neuropsychologist and Rehabilitation Assistants working the Southampton and West Hampshire areas.Working Pattern: Monday - Friday with the expectation for some weekend working.A full and valid UK driving licence and access to a vehicle for work purposes is required.
Main duties of the job The Neuro Early Supported Discharge Team provides up to six-week of intensive neurorehabilitation to individuals with a Stroke or Neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital and have a GP within Southampton City or parts of West Hampshire.
The multidisciplinary team comprises of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Nursing, Clinical Psychology and Rehabilitation Assistant therapy, which is delivered in a client's own home or alternative community setting.The Neuro Early Supported Discharge Team works closely with all services involved in the Stroke and Neuro Pathways in Southampton, including University Hospital Southampton Hyper-acute Stroke and Acute Stroke Units and the Wessex Neuro Centre, as well as sister teams within the Neuro Rehab Services- Snowdon Neuro Rehab Unit, Community Neuro Rehab Team, Vocational Rehabilitation Service, Neuro Rehab Gym, Transition Team, Psychology and Speech and Language Therapy team.Intervention from the Early Supported Discharge Teams aims to facilitate improved independence and quality of life based upon agreed personalised rehabilitation goals.The purpose of this position is to provide Senior Rehab Assistant intervention to a specified caseload of patients as part of a comprehensive multidisciplinary team, delivering intensive neuro-rehab intervention programmes based upon individualised patient goals.

Person Specification Qualifications Essential Foundation Degree / Diploma or NVQ3 with experience equivalent to a foundation degree level.
Desirable Neurological specific training equipment, gait re-education, handling, Neuro/Stroke Rehabilitation, LTC management, Palliative care, Postural management, Continence Care.
Experience Essential Experience of regular assessment and treatment of patients presenting with problems requiring therapeutic intervention, some of a complex nature.
Recent NHS experience of treating people with a range of stroke / neurological needs continence, mobility, balance problems, cognitive deficits, dysphagia.
Relevant experience of working within a multidisciplinary team in either a hospital or community setting (some of which has been gained within a UK setting).
Working within a multiprofessional team.
Desirable Supervision and training of others.
Involvement in regional or national clinical bodies clinical interest groups, conferences etc.
Additional Criteria Essential Demonstrate strategies for effective organisation of own workload and that of junior staff on a day-to-day basis.
Demonstrate and provide evidence of skill development and provide examples of good practice where key skills have been applied.
Evidence of application of the rehabilitation process.
Ability to act on own initiative and take responsibility contacting a senior clinician / other service when a client requires urgent attention.
Able to use strategies to deal with difficult situations and appropriately seek support and advice when required.
Shows flexibility and ability to work within a demanding and changing environment.
Strong computer skills Word, Outlook.
Demonstrate strategies for problem solving and clinical decision making.
Good verbal and written skills pleasant manner when speaking to others, clear and concise messages delivered, legible handwriting.
Able to communicate sensitive information in an appropriate and effective manner.
Knowledge and understanding of the stroke rehabilitation process.
Understanding of clinical governance requirements and demonstrates compliance.
Understanding of health & safety responsibilities and demonstrates compliance.
Understanding of data protection issues and the need for confidentiality to be maintained.
Is able to work legally in the UK.
Is able to work with children and vulnerable adults.
Ability to travel daily for meetings across a range of sites in Southampton/Portsmouth/Hampshire.
Access to transportation on a daily basis.
Full UK driving licence if using car and willing to insure for business use.
Ability to work weekend, weekday and evening shifts.
Desirable Experience of participating in audit.
