Job summary Are you looking for a new challenge where you can make a difference in the lives of vulnerable children and young people as they reach 18 years where there are multi-agency concerns about their ability to access emotional wellbeing and mental health support which could significantly enhance their ability to reach their full potential.
Young people who also require transition to adult community services in a smooth and streamlined way until they reach 18 years and 3 months.
We have an exciting opportunity for an experienced Senior Psychological Wellbeing Practitioner to join Surrey & Borders NHS Foundation Trust in the newly developed Surrey Emotional Wellbeing and Mental Health Service.
As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations you will be based in our newly transformed Reaching Out service.
The Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network approach.
You will be a part of the Transition Pathway within the Reaching Out Service which provides support for 17 and 18-year-old's as they transition out of Children's EWMH Services.
Main duties of the job You will work with young people who have been receiving treatment within Children and Young People's Service (CYPS) Community Teams and may experience some anxiety about the transition.
Some of them may require further support from CMHRS as they turn 18, and others may benefit from short-term low-intensity interventions and signposting to gain more confidence in managing their difficulties.
You will offer treatment interventions following the principles of IAPT-compliant, evidence-based low-intensity treatment protocols based on Cognitive Behavioural Therapy.
Our aim is to maximise young people's engagement with services and help them move forward.
You will have experience post qualification in Low Intensity Psychological Therapies and demonstrable expertise working with young people.
A key part of this role is building relationships and trust with young people.
An important requirement of a successful candidate is the ability to creatively adapt their approach to engage with each individual and be approachable, friendly, and tenacious.
Through building a trusting relationship, the Reaching Out Psychological Wellbeing Practitioner will be able to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services.

We also provide social care services for people with a learning disability in Croydon and ASD and ADHD assessment services in Hampshire.
We actively seek to engage people who use our services and our communities in improving the mental wellbeing of the local population.
We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.

For international travel, both Gatwick and Heathrow airports are nearby.
