Job summary An exciting opportunity has arisen working with Havant and Waterlooville CMHT at Parkway in Havant.
The post holder will be from a mental health nurse background.
The role will involve care coordination of service users who have severe and enduring mental health problems.
There is a strong multi disciplinary team approach where risk is shared ensuring good support for the successful applicant.
The team work closely with our colleagues in psychology therapies and with partner agencies including housing, police, county council and primary care.The role requires lone working, use of initiative and a commitment to team working and service user empowerment.
We offer regular supervision and a variety of specialist therapy training opportunities.
There is also opportunity to access additional training through the ACE academy.You will need to be able to work flexibly and to demonstrate knowledge of CPA process, risk management and the recovery model.
You will also need to establish good working relationships with the local Primary Care Networks, NHS Talking Therapies Hampshire and other community and voluntary organisations.A car driver is essential with access to own car.Open only to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service or Hampshire Hospitals FT. Main duties of the job The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSW's where appropriate.Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
