This role is perfect for an experienced physiotherapist with a passion for delivering high-quality care and contributing to the discharge planning process.
Key Responsibilities: Provide expert physiotherapy assessment and treatment for patients on acute inpatient wards.
Lead and coordinate the discharge planning process, ensuring safe and effective patient transitions from hospital to home or other care settings.
Work collaboratively with multidisciplinary teams to ensure holistic care delivery.
Develop and implement individualized rehabilitation plans based on patients' needs and clinical conditions.
Supervise and mentor junior physiotherapists and support staff.
Maintain accurate and up-to-date patient records, ensuring compliance with professional standards and best practices.
Participate in regular clinical supervision and continuous professional development activities.
Key Requirements: HCPC registration is essential.
Recent experience in an acute inpatient ward setting is required.
Discharge planning experience is essential, demonstrating the ability to plan, assess, and execute safe patient discharges.
Ability to work autonomously and as part of a multidisciplinary team.
Excellent communication, organizational, and clinical skills.
