Senior Nurse - Mental Health And Learning Disabilities - Hmp Berwyn

Job summary An exciting opportunity has arisen in the mental health and learning disabilities services for an experienced, motivated and forward thinking practitioner to provide specialist substance misuse care within the custodial environment, working as part of an integrated and multi-disciplinary health service.
You will be expected to deliver a range of specialist interventions at secondary care level to include referral management, screening assessment, triage and evidence based interventions by providing one to one and group facilitated sessions whilst managing a mixed and challenging caseload.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job Duties will include performing robust assessment, screening and interventions to offenders with substance misuse concerns and ensuring that all waiting times, assessments and interventions are delivered in a timely manner in line with Mental Health Measure Wales.
As a named care coordinator you will need to be proactive, using an evidence-based approach to clinical interventions, health promotion/ prevention activity and the management of acute and long terms conditions using a stepped care approach.
You will ensure integrated and collaborative working with other specialist services, including the continuity of care, liaising with community mental health teams to ensure appropriate sharing of information and provide comprehensive and timely reports and mental health expertise to Multi Agency Public Protection Arrangement panels (MAPPA) police, probation services, solicitors and court liaison teams as directed.
About us We are looking for a dynamic and enthusiastic practitioners with a real interest in forensic mental health and substance misuse and whilst experience in a forensic setting is desirable, it is not essential.Our aim is to identify staff with a strong belief in recovery and who are comfortable working closely with forensic prisoners.
This type of setting will provide challenges and it is important that you have good understanding of caring for patients with acute mental health backgrounds.
