Job summary This is an exciting opportunity for a motivated Occupational Therapist to join a dynamic multidisciplinary community rehabilitation service in Southampton.
The Community Independence Service is integrated with Community Nursing and forms the West Locality, delivering programmes of rehabilitation working in partnership with other social and healthcare providers.The role will allow individuals to gain experience in falls assessment, postural management, education and prevention, complex community rehabilitation and goal focused rehab planning.
Additionally, the role will involve provision of equipment and minor adaptations to support independence, providing advice on health and wellbeing and signposting to other community services.
This will be supported by therapy staff, an advanced clinical practitioner and geriatricians.The team promotes inter professional working which supports the single assessment process for patients.
There is a robust supervision and appraisal framework within the team and opportunities for training and development.
You will need effective interpersonal skills and to demonstrate skills to support a flexible and positive approach to problem solving.

Main duties of the job Work as part of the integrated locality health and social care team delivering programmes of rehabilitation within a community setting in close collaboration with other relevant services/agencies other community and inpatient teams, social care, primary and secondary care.Manage a caseload with supervision as required/be responsible for a defined area of service, determining daily occupational therapy and multidisciplinary rehabilitation intervention, and able to plan and implement comprehensive programmes of care for people who require intensive support / rehabilitation, guided by more senior clinicians where required.Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment rehabilitation assistants and students on placement.

Person Specification Qualifications Essential Degree or Diploma in Occupational Therapy, state registered.
Recognised Clinical Educator for Occupational Therapy students.
Desirable Post Registration Course rehabilitation, chronic disease management, palliative care, postural management.
Experience Essential Substantial post qualifying experience of working with vulnerable adults with complex presentation which includes older people.
Significant experience of treating people with a range of occupational therapy needs poor posture, poor upper limb function, memory problems.
Significant range of relevant experience in either hospital or community setting including working autonomously.
Experience of supervision and training of Rehab Assistants and students.
Working within a multi-disciplinary team.
Demonstrate complex occupational therapy skills functional assessment of daily living skills, cognitive and perceptual assessment.
Demonstrate good time management skills and strategies for organising own workload and that of junior staff day to day, prioritising and delegating appropriately.
Desirable Experience of participating in an audit.
Knowledge of leadership strategies, group dynamics and change management
