Job summary We are currently looking to recruit an enthusiastic andmotivated senior community nurse to join our team.
The successful candidatewill be based at Pershore Community Hospital and will provide leadership and supportto their colleagues working within the Pershore and Upton Neighbourhood Team.
We are looking for a passionate individual who isexcited by change and is keen to deliver nursing care in the community.
Thesuccessful post holder will be triaging calls that are coming into theNeighbourhood Team.
You will be able to achieve a balance of triaging andpatient facing visits, as you will also be visiting patients within their homeenvironments, conducting holistic assessments for our Intensive Supportpatients who are coming out of hospital via Pathway 1, or completing anAdmission Prevention assessments.
We are a supportive team who are committed todeveloping services to provide individual care for our patients.
We offerdedicated time for training and clinical supervision, with a supportive L&DDepartment to help guide.
We will provide you with resources to develop andprogress, as well as flexibility, professional development and a wide range ofwellbeing and support.

Main duties of the job The role will require:Workingflexibly to provide the service delivery need across the local area (theservice operates from 7:30am 6:30pm, Monday to Sunday including bankholidays)Anenthusiastic, motivated and innovative thinker to become part of a primary carefacing communityTheability to work autonomously and manage your own work load and work within amulti-disciplinary teamExperienceof working in a fast paced and varied environmentReceiving calls for district nursing,occupational therapy and physiotherapy patients.You will use your clinical knowledge totriage these phone calls which may either result in a same day visit, or adviceand guidance can be given to promote self-care and independence.
You will workalong side administrators who will also be receiving phone calls, and as theretriaging clinician you will provide support and guidance to them also.
You will manage and oversee the IntensiveSupport caseload, undertaking nursing assessments and reviews (72hours and 1week), to maintain your own holistic assessment skills.
You will be involved in team meetings,goal/care planning and evaluation and handover meetings.
You will also supportthe teaching and learning of nurse students pre and post registration.

Work as an autonomous practitioner with the integrated care teamproviding rapid and planned expert professional assessment and treatmentinterventions to patients in their own homes or other community settings toenable patients to maximise their independence, health and wellbeing and remainin their own homes for as long as possible.
This will be achieved through acombination of hands on expert clinical skills and knowledge to treat, educateand support patients on a designated caseload and by working with patients andtheir carers to assess, plan and implement appropriate packages of care.
Demonstrate expert clinical knowledge, health promotion andholistic assessment skills and will work with the patient and their carers toassess, plan, implement and evaluate their ongoing care and support needs intheir own home or temporary/usual place of residence.
Support the Operational and Clinical Team leaders in the deliveryof best nursing practice, undertake triage, and an involvement with the allocationof work and leadership of other staff, manage team resources and deputise intheir absence.
