Senior Clinical Pharmacist - Community Response Services

Job summary We are seeking an enthusiastic, patient-focused, independently motivated pharmacist to join our Community Response & 2 hour Urgent Response Teams.
The post holder with be co-based with the teams, currently at Cobalt Exchange in North Tyneside.
We provide a proactive and collaborative, high quality medicines optimisation service for patients in their own homes working closely with colleagues in health and social care across both the primary and secondary care sectors.
The role will provide a mix of face to face, patient-centred consultations and telephone support to patients, family members and carers as well as involvement with multidisciplinary team meetings.
In addition, provision of responsive medicines management advice and support within the 2HUCR service providing support to patients with urgent medication needs.
We need an experienced pharmacist with a clinical diploma or equivalent clinical pharmacy experience, and a non-medical prescribing qualification would be preferable.
The job role will also integrate into the pharmacy team weekends and on-call rotas unless agreed otherwise in writing on appointment.
Main duties of the job Working within professional boundaries to provide person-centred, evidence-based structured medication reviews for residents in their own homes.Targeted reviews of frail elderly patients in their own homes to help minimise risk at transfer of care including those patients who have recurrent falls,complex polypharmacy, swallowing difficulties and those taking high risk medicines.Close working with the multi-disciplinary team, which is likely to include attendance at MDT meetings.
Interacting with a wide variety of health/social care professionals including; GPs; community nurses/matrons; community therapy teams; specialist secondary care consultants (, geriatricians, Parkinson's disease specialists); the old age psychiatry team; and pharmacy colleagues in community pharmacy, GP practices and the hospital setting.Patients are reviewed holistically to identify and classify their medication risk and pharmaceutical care plans are implemented to reduce their risk of poor outcomes from polypharmacy and inappropriate prescribingProvision of expert advice, information and recommendations on medicines and prescribing, including support to NMPs within the service, providing clinical governance in areas such as antimicrobial and opioid stewardship.The role is patient-facing, with reviews conducted through a combination of telephone, face-to-face and video consultations with patients/residents and their families/carers.

Job description Job responsibilities To lead, deliver and support ongoing development of a clinical pharmacy service within the Community Response Service (CRS) and the Community Services Business Unit (CSBU) of the Northumbria Healthcare NHS Foundation Trust.To provide excellent patient care through direct patient facing activities as part of clinical pharmacy service delivery within CRS.To provide leadership on matters pertaining to medicines use and optimisation within the CRS (and in support of the CSBU).
This includes providing advice on medicines use (risk, clinical governance, research and development, formulary, expenditure) and to contribute to the development of relevant protocols and guidelines.To undertake risk management and clinical governance activities within own assigned areas of responsibilityPharmacy services provided within the Community Services Business Units, within which the Community Response Service falls.
This role encompasses the 2hr Urgent Response Service, Admission Avoidance, Proactive Care, Community Therapy and provide support to Virtual Wards when necessary.

Person Specification Qualifications Essential 1st degree - Vocational masters degree in pharmacy Previous pre-registration training Registered with General Pharmaceutical Council (GPhC) Postgraduate clinical qualification (over and above that of the 1st degree, preferably at Masters level) and/or significant relevant clinical experience to demonstrate a similar level of knowledge Evidence of continuing professional development to at least meet the mandatory requirements of the GPhC IT skills to at least ECDL standard or equivalent experience Independent prescribing qualification or commitment to achieving following appointment.
Desirable Member of Royal Pharmaceutical Society and/or other professional groups to facilitate continuing professional development Experience Essential Significant and relevant post registration experience Knowledge and experience of the operation of NHS Organisations Experience of supervising staff Other Essential It is an essential requirement of the role that the post holder has a valid driving licence
