Job summary Based both at Winsley, and out in our community.
You will line manage and support an allocated group of Hospice at Home Carers, including day-to-day coordination, performance reviews, and addressing concerns or complaints from patients, families, or professionals.
We have both full and part time position available with various shifts.

You will line manage and support an allocated group of Hospice at Home Carers, including day-to-day coordination, performance reviews, and addressing concerns or complaints from patients, families, or professionals.
The role will include: Providing support for Carers during their shifts via telephone or in person, ensuring they have the resources and guidance needed to perform their duties effectively.Conducting face-to-face assessments of patients, including care planning and risk assessments, ensuring accurate documentation and patient-centered care.Collaborating with the Deputy Hospice at Home Manager to monitor and review cases, carrying out quality assurance audits to maintain high standards of care.Performing care visits when necessary to support safe service delivery and avoid unmet needs, adjusting care plans based on ongoing assessments and patient needs.Communicating regularly with Nurse Specialists and the wider Multi-Disciplinary Team to ensure timely and optimal care for patients and families, processing referrals and maintaining accurate records.Participating in the planning and provision of educational programs for the Hospice at Home Care Team, supporting the training and competency development of new and existing staff.Engaging in professional development, attending supervision sessions, participating in performance reviews, and adhering to Dorothy House policies and procedures to ensure high-quality care and continuous improvement.
