Due to the continued growth of our organisation, we're seeking a new dedicated coordinator who will aid in supporting our clients to maintain their health and enhance their independence.
The role: As a Senior Care Coordinator within The Superior Healthcare Group's Complex Care Division, you'll join our team of experienced coordinators and will manage your own client caseload and team of coordinators.
Working 37.5 hours per week, your role will involve collaborating with clients, their families, carers, and the wider coordination team, to cover the shift requirements for our clients.

Our Senior Care Coordinator will offer thorough support to ensure you are fully prepared to coordinate high-quality, safe, and compassionate care.
You will receive a salary of £28,350, which will increase to £31,500 upon successfully completing your probation.
About Superior Healthcare: Superior Healthcare was established in response to our founders' personal experience of caring for a family member who has an acquired brain injury.
Today we work as a specialist provider in complex care at home, with a full nurse-led service, providing care for children, young people and adults who have long-term health conditions.

You will be responsible for specific clients, and maintain an excellent working relationship with them.
Working with staff to establish effective employer-employee relationships, minimising legal risks.
Proactively address day-to-day events and maintain high standards in communication and event management.
Play a key role in implementing and maintaining Superior Healthcare's quality assurance programme, ensuring compliance with policies and safeguarding procedures while promptly addressing any concerns raised.
Requirements Previous Care Coordination experience.
Genuine interest in working within a caring environment.
Excellent interpersonal skills and a 'team player'.
Ability to work autonomously and as part of a wider team.
Observant and sensitive to a carers and client's needs.
Excellent oral and written communication skills.
Good IT skills including use of Microsoft Teams and Outlook.
