Job summary An exciting opportunity has arisen within WarringtonInnovation Network to recruit a Senior Clinical Pharmacist to lead our PCNPharmacy Hub team.We are looking for a dynamic and skilled Clinical Pharmacistwho may already be established in a leadership role or who would be interestedin developing their career in a leadership role.
You will be joining a team ofpharmacists and pharmacy technicians and be responsible for the leadership, managementand training of the pharmacy team to ensure delivery of exceptional care forour patients.
Main duties of the job The role of PCN Senior Advanced Clinical Pharmacist is avaried role, essential to the day-to-day operation of our PCN Pharmacy Hub.
Therole will be crucial in supporting the delivery of effective, safe and highquality of pharmacy services to the PCN population.
The post holder will be thefirst line of support and seniority for all pharmacy team colleagues on siteand will be responsible for a range of clinical and leadership duties.

Our member practices include; 4Seasons MedicalCentre, Chapelford Medical Centre, Culcheth Medical Centre, Guardian MedicalCentre, Parkview Medical Centre & Springfields Medical Centre Job description Job responsibilities Clinical PharmacyTo take referrals and triage them from a widerange of health and social care professionals to develop pharmaceutical supportpackages for patients with complex medication needs in order to ensure they arecompetent in the use of their medicines.Undertake patient facing clinics independentlywithin the scope of your prescribing competency Undertake medication reviews of PCN patientsensuring involvement of the residents, relatives, care home staff and GPpractices and liaising with the GP, Community Pharmacist, and Care Home Managerregarding the outcomes.Proactively lead initiatives that promote a goodquality service.
These include:Intervention monitoring and reporting forprescribing alertsMedication error reporting.Clinical audit and research projects.Reporting adverse drug reactions to theCommittee on Safety of Medicines via the yellow card reporting scheme.Promote adherence to National Patient SafetyAgency advice.Investigation of complaints and incidentsrelating to pharmacy teamMonitoring, inspection and destruction relatedto controlled drugs within primary care.
Provide specialist clinical knowledge and expertadvice on complex prescribing related issues where requested.Deliver elements of clinical governancestandards related to prescribing and medicines management contained withinDepartment of Health guidance.
Encourage safe repeat prescribing in thepractices by facilitation development of protocols, audit and feedback.To provide specialised medicines and therapeuticadvice to inform GP practices on priorities and actions, and the treatment ofpatientsManagementProvide Monday to Friday, 8:30am 4:30pmon-site clinical seniority presence and support within our PCN Pharmacy hub To line manage, mentor, lead and supervise otherpharmacists and technicians employed by WIN PCNTo manage performance, competency assessment andprofessional development through clinical audit, appraisal process and one toone meetings.
To be a role model for the pharmacy team andactively demonstrate the organisational and departmental values and behaviours.
To challenge behaviour that is not in line withWIN PCN values and help make improvements, through coaching and mentoring.To have direct staff managerial responsibilityif required, including managing performance and professional development.Provide timely, clear and concise writtenevidence of HR related interactions to the PCN Strategy ManagerLeadership To be the clinical lead pharmacist forWarrington Innovation Network To provide daily operational leadership to manageand prioritise workload amongst pharmacy team members Laise with the PCN Operations Lead / PCNStrategy Manager around any safety concerns regarding workload Liaise with the PCN GP Clinical Safety Leadaround any safety concerns regarding workloadTo lead on the development of a positive safetyculture within the pharmacy teamWork with the senior leadership team andclinical safety lead to implement the PCN Safety StrategyTo lead on the use of an evidence-based approachto influence prescribing and ensure patient-centred and cost-effective choiceof therapy.To lead on clinical audit and projects for theteam to support safe medicines use and prescribing for the PCN.Research and AuditLead and report on clinical audits andimprovement projects identified within WIN PCN and member practices Support the safe use of antimicrobials andcompliance with policies and procedures relating to medicines and riskmanagement processes.Evaluate interventions and action plansimplemented, for effectiveness, re-audit and research.
Critically analyse research findings and theirimplications for practice.Training and EducationTo provide an ethos of continuous learning anddevelopment within the PCN pharmacy team Provide an annual training schedule for thepharmacy team for protected learning time eventsTo deliver clinical education and trainingsessions for the pharmacy teamSupport/arrange for other pharmacy team membersto deliver educational sessions within the team Attend member practices to deliver education andtraining on medicines issues and support/arrange for other members to delivereducational sessionsTo support the development of the pharmacy teamthrough clinical supervision, education and training as required.Observe telephone and face to face sessionsconducted by the pharmacy team to check competencies and support learning anddevelopment.
Maintain an up-to-date competency matrix for allmembers of the pharmacy team.
To be involved in the supervision and trainingof healthcare professionals where there is an identified need for providingsupport for medicines Provide timely, clear and concise writtenevidence of any learning/training needs and supportive action takenTo take responsibility for own personaldevelopment and continuing education and training as necessary to ensure a highlevel of competency required in carrying out tasks.To remain aware of national developments inpharmaceutical care and undertake Continuing Professional Development inaccordance with the requirements of the General Pharmaceutical Council.LocationDuring working hours, the individual will be required to bebased on site at our WIN PCN Pharmacy Hub or from other work locations withinthe Warrington area as required to meet service demands/needs of the business.
Given the geographical nature of the PCN, you will berequired to travel between member practice premises and multiple communitylocations as part of your role.
Person Specification Skills Essential Specialised knowledge of drugs used and therapeutic trends within specialised area Excellent oral and written communication skills Ability to influence, negotiate and practice at senior level High level of report writing and presentation skills Keyboard skills and computer literate, with the ability to use software packages.
Excellent interpersonal and negotiation skills in order to communicate confidently and credibly in all situations Demonstrates awareness of and commitment to the Clinical Governance agenda and policy development Demonstrates awareness of the national agenda on new models of healthcare Keen to undertake self-development Ability to work under pressure, prioritise and to meet deadlines Ability to work independently and as part of a team Ability to effectively manage occasional distressing or emotional circumstances distressed patients Good mentoring, coaching and relationship building skills Self-motivated and confident Demonstrates an empathetic, compassionate nature Qualifications Essential Pharmacy Honours degree (BPharm or MPharm) Postgraduate Clinical Pharmacy Diploma Registration with the General Pharmaceutical Council (GPhC) Non-medical prescriber (Pharmacist Independent Prescriber) or willingness to complete the NMP course to attain the necessary qualification Other requirements Essential Current UK professional registration Disclosure Barring Service (DBS) check To be based within PCN Pharmacy Hub office Ability and willingness to travel across all PCN sites, community locations and to patients own homes as required Ability to work in hot desk areas as and when required Flexible approach to work patterns in line with needs of the service Experience Essential Substantial post registration experience in Primary Care Experience of multidisciplinary team working and making clinically appropriate interventions Demonstrable experience of cross sector collaborative working with primary care healthcare professionals Experience of writing and reviewing policies, guidelines and SOPs Experience of service development Experience of line management and clinical supervision Experience and competence to provide patient information & education to patients, families and carers Experience of undertaking clinical audits and improvement projects leading to service improvements Ability to demonstrate an understanding of the key stakeholders the post is likely to be supporting and how they will be expected to support them.
