We have GP Vacancies at our Health Board Practice, West End Medical Centre, in Conwy County.
West End Medical Centre is situated in Colwyn Bay, with a list size of 18,400 patients.
Newly merged from two practices within the same building, West End Medical Centre is focused on delivering a social model of care and has a multi-disciplinary team including GPs, Nurses, Physiotherapists, and Pharmacists.
The practice is also moving towards recruitment of other health care professionals such as Advanced Practice Paramedics and Physicians Associates.
Main duties of the job In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
In accordance with an agreed practice timetable, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone, video, and email consultations and queries, visiting patients at home, providing advice to other colleagues, and dealing with queries, paperwork, referrals, and correspondence in a timely manner.
Make professional, autonomous decisions in relation to presenting problems, including undifferentiated and undiagnosed problems, whether self-referred or referred from other health care workers within the organisation, with due reference to best practice and evidence-based guidelines.
Screen patients for disease risk factors, early signs of illness, and promote public health interventions.
Admit or discharge patients to and from the active caseload and refer to other care providers as appropriate.
Record clear and contemporaneous consultation notes to agreed standards.
Collect data for audit purposes.
About us The Location: Conwy County and Colwyn Bay both have a variety of independent coffee shops and bars and are close to the town of Llandudno with a good range of retail stores and a theatre and conference centre.
Its coastal location also means that within minutes you can be in the countryside and heading to the Snowdonia National Park.
There are also good road and rail links to Chester, Manchester, and Liverpool.

Person SpecificationPersonal Qualities Flexible approach Motivated and efficient Commitment to learn and best practice General Satisfactory Immigration Status Satisfactory Health Clearance Satisfactory Disclosure Check Valid Driving License Car owner Qualifications Full GMC Registration with a licence to practise and entry on the GP Register Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent Included on a Medical Performers List or eligible Evidence of continuous professional development MRCGP Experience Experience in general practice and working in a multi-disciplinary team Vocational GP training Eligibility for minor surgery, obstetrics, family planning accreditation Skills and Knowledge Ability Proven ability to handle a busy and varied primary care caseload and respond flexibly to workload fluctuations Ability to take independent clinical decisions when necessary and to seek advice from senior doctors as appropriate Commitment to team approach and multi-disciplinary working Effective counselling and communication skills Understanding of clinical risk management and clinical governance Commitment to participating in and understanding of the management process Ability to develop and sustain relationships with a wide range of individuals and within groups Ability to work across organisations to deliver a common objective Ability to draft letters, reports, and protocols in a timely manner Audit Research interests relevant to the post Teaching / Training experience Ability to speak Welsh Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
