Job summary We are looking to recruit two Salaried GP's who will work within our west locality which will cover The Willows Medical Practice, Thornton Medical Practice and Denholme Medical Practice.
We are recruiting for 10 sessions with the working hours between 08:00am - 6:00pm.
The postholder, acting within theirprofessional boundaries, will provide care for the presenting patient frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of care.
They will demonstrate safe, clinical decision-making andexpert care, including assessment and diagnostic skills, for patients withinthe general practice.
They will work collaboratively with the general practiceteam to meet the needs of patients, whilst supporting the delivery of policyand procedures.
Main duties of the job Assess, diagnose,plan, implement and evaluate treatment/interventions and care for patientspresenting with an undifferentiated diagnosis.

Assess , diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
Undertake on-call duties for clinically urgent and emergency problems and consequently be included in the doctors on-call- rota.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of with patients in order to support compliance with and adherence to prescribed treatments.Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
Prioritisehealth problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation.
Ensureappropriate and safe follow up of patient where treatment initiated and/or additional investigations requested.Recognise and workwithin own competence and professional code of conduct as regulated by the GMC.Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures.Prioritise,organise and manage own workload in a manner that maintains and promotesquality, including clearing clinical tasks on a daily basis, filing pathologyresults in a timely manner and re-setting recalls appropriatelyDeliver careaccording to NSF, NICE guidelines, Practice Protocols and evidence-based careAssesseffectiveness of care delivery through self and peer review, benchmarking andformal evaluation including appraisal/revalidation processInitiate andparticipate in the maintenance of quality governance systems and processesacross the organisation and its activitiesUnderstand andapply legal issues that support the identification of vulnerable and abusedchildren and adults, and be aware of statutory child/vulnerable patients healthprocedures and local guidanceUndertake at least onefull audit cycle per year on a topic that is of interest to the practice andpresent your findings to the teamFollow the practiceformulary for prescribing and not routinely be an outlier in prescribingbehaviour Prescribe in linewith KPPIsAttendance at 80%of the MDT when it is your routine day of workingWork closely withthe Patient Service Manager, other Associate Partners and Responsible Partnersat the practice to form a cohesive practice management team to ensure thatpractice achieves highly on all its performance indicators.Contribute todiscussion and constructively challenge and add to debate, bringing new ideasto the table and be an active member of the partnership teamTo routinely attendthe PLTs and take an active role in those events Share best practiceand innovation within your immediate team and help cascade such practice throughoutthe whole organisationSupport the PSM inmanaging complaints effectively in an environment of openness and willingnessto learn from patient feedback Person Specification Personal and people development Essential Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role mode Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Share learning and information gained to other team members in order to share good practice and inform others about current and future developments (eg courses and conferences) Assess own learning needs and undertake learning as appropriate Critically evaluate and review innovations and developments that are relevant to the area of work Take a role in planning and implementing changes within the area of care and responsibility Contribute to the development of local guidelines, protocols and standards Maintain effective communication with those responsible for the overall commissioning and procurement process Maintain active involvement in the planning and processes of practice-based commissioning or similar initiatives Desirable Contribution to clinical meetings Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence Prioritise own workload and ensure effective time-management strategies.
