Job summary This is a position for a newly qualified, enthusiastic,and forward-thinking GP to join the Upper Calder Valley PCN, Calderdale within West Yorkshire.
Currently the role is fixed term up until 31stMarch 2025.As this post is funded through the Network Contract DES,you must be within 2 years of GP qualification and not have been substantivelyemployed as a GP previously.This post will require you to work across differentpractices within the PCN all of which are rated as Good with CQC.
This presents a great opportunity to gainvaluable experience and insight of the workings and culture within differentpractices.We are a Sponsor organisation for the employment of Skilled Workers.A training budget of £3,000will be invested in your development during this contract.
We will work with you to tailor a package oftraining support and study leave that will give you the necessary skills,knowledge and understanding to thrive in these early years of your career.Between 4 and 8 sessions perweek (full time, part time please state the number of sessions as a preference in your application) Main duties of the job You will manage a case load and will work autonomously ona day-to-day basis.
To support your development,you will have a dedicated GP assigned to carry out professional supervision.You will undertake several duties governed by the GMScontract including:Face to face and telephone consultationsPatient home visits and carehome visitsChecking and signing repeat prescriptionsAdministrative tasks namely dealing withqueries, paperwork and correspondence as requiredCover all clinical management of in-surgerypatients as appropriate including initiating investigations, reviewing resultsand making referrals to secondary care or to other providers as appropriateDevelop care and treatment plans in consultationwith patients About us Pennine GP Alliance is a member-led organisation made up of 19 GPpractices in Calderdale who form 5 Primary Care Networks (PCNs), collectivelyserving over 223,000 registered patients.We work extremely closely with general practice but also inpartnership with other local healthcare providers with a purpose to ensureviable GP services remain at the heart of local communities, providingsustainable and high-quality patient-focused healthcare.Join our team and experience a workplace that truly values growth,collaboration, and impact.

Job description Job responsibilities In accordance with the PCN/Practice timetable,as agreed, you will make yourself available to undertake a variety of dutiesincluding surgery consultations, telephone/video consultations and queries,visiting patients at home, checking and signing repeat prescriptions anddealing with queries, paperwork and correspondence in a timely fashion.Making professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care professionals within the Practice or external organisations.Assessing the health needs of patients withundifferentiated and undiagnosed problems.Screening patients for disease risk factors andearly signs of illness.Contributing to QOF requirements and recordingdata appropriately.Contributing to PCN requirements and recordingdata appropriately.In consultation with patients and in line withcurrent practice disease management protocols, developing care plans forhealth-related illnesses.Screening patients for health education.Admitting and discharging patients to and fromthe caseload and referring to other care providers as appropriate.Recording clear and contemporaneous consultationnotes to agreed standards.Collecting data for audit purposes andcontributing notes to the practice audit programme as required.Compiling and issuing computer generated acuteand repeat prescriptions.Prescribing in accordance with the acceptedprescribing formulary whenever clinically appropriate.Take responsibility for some aspects of theclinical work within the team including co-ordinating aspects of governance,QOF, enhanced services, CPD and keeping clinical protocols up to date.Undertake all normal duties and responsibilitiesassociated with a GP working within primary care, as set by Royal College ofGeneral Practitioners, General Medical Council, all other Royal Colleges andcouncils applicable to the staff groups working within the Practice, Departmentof Health, Integrated Care Boards and all other regulatory and standard settingorganisations.
Person Specification Qualifications Essential Must be a qualified GP within 2 years of registration.
MRCGP Full GMC registration Performers List registration No previous substantive posts as a GP prior to this application Clear Enhanced DBS check UK driving licence with access to own car Experience Essential Experience of working within the NHS and with the general public Working with EMIS and/or SystmOne Well developed IT skills and an enthusiasm for new applications and software Good time management and highly organised Ability to use initiative, judgement, resourcefulness, common sense and local knowledge Interest in quality improvement work Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate Able to work under pressure and to meet deadlines Produce timely and informative reports Work effectively independently and as a team member Demonstrates accountability for delivering professional expertise and direct service provision Desirable Working knowledge of the NHS and care system in Calderdale Understanding the demographics and needs of patients locally
