Job summary Fountains Medical Practice has a growing practice populationand is based in Fountains Health.
In addition to providing anexcellent primary care service to our patients, we provide a successful serviceto all the care homes in Chester city through the Fountains Integrated CareHomes Service (FICHS).
The FICHS team works with the multidisciplinary servicesin the community, secondary care specialists and voluntary sector to provideholistic care to patients in care homes.
We work closely with other practices as partof a successful and expanding PCN and we are a training practice, working with PhysiciansAssociate students and Medical Students from Chester University and theUniversity of Buckingham.
Main duties of the job Due to continued expansion, an exciting opportunity hasarisen for a salaried GP for 4-6 sessions per week to join our Care Home Team.
We would ideally expect the successfulcandidate to have experience of managing patients with long term conditions,acute illnesses, a working knowledge of QOF and the current Direct EnhancedServices.
Experience or interest in care of the elderly and palliative care isdesirable.

The candidate must have excellent clinical and interpersonalskills to ensure safe, effective and high standards.
They should be willing to work hard todevelop the role to compliment the clinical and administrative support staff.
