Primary Responsibilities:Completes assessment of patient during visits for effective symptom management.Perform treatments/interventions in accordance with the Plan of Care and physician's orders.Provides emotional support to patient/caregiver.Teaches patient/caregiver as appropriate to plan of care.Follows Standard Precautions.
Reports to Triage Nurse following patient visits.Leaves accurate and detailed messages in voice mail boxes for all appropriate team members after making all visits including death visits.Accurately completes documentation of visits/phone communications and submits documentation in a timely manner per policy.Calls physician as necessary for change in Plan of Care.Facilitates change to continuous care when necessary as per policy.Follows policy when patient is transferred to acute care after hours.Arranges for medications and supplies in an emergency per policy.Answers pager and/or phone calls within 15 minutes.

Typical qualifications would be equivalent to:Possession of a degree from a diploma nursing program, 2 year Associate Degree, or Bachelor's Degree in Nursing.Minimum of two (2) years experience in acute care setting, oncology, home health setting, and hospice preferred.Knowledge of principles and practices of nursing.
