Primary Responsibilities:Weekend coverage:Performs skilled nursing activities such as:Completes assessment of patient for effective symptom management.Incorporates nursing skills, communication skills, and emotional intelligence skills to effectively assess problems/needs.Perform treatments and interventions in accordance with the plan of care and physician's orders.Provide information and education regarding disease progression and anticipated needs at end of life.Provide instruction to the patient/care giver in accordance with the plan of care or changes in plan of care.Collaborate with patient, family/caregiver, and practitioner regarding goals of care and changes in treatment plan.Follows precautions based on plan of care.Provides appropriate emotional support during all conversations with patients/caregivers.Conducts follow up and collaboration calls as needs arise or as requested by the team.Minimum Qualifications:Possession of a degree from a diploma nursing program or a Bachelor's degree in Nursing.Minimum of two (2) years experience in acute care setting, oncology, or home health care setting.Skilled at teaching, communication and listening skills, assessment skills, central/peripheral IV skills, working within the framework of a multidisciplinary team.Required Licenses/Certifications: Current license as a Registered Nurse in the State of Florida.Current CPR certificationCurrent State of Florida driver's license with driving record acceptable to Community Hospice of Northeast Florida insurance carrier and reliable transportation.Physical Requirements/Work Environment:Home care position for 8 to 12 hour shifts.
Use of own vehicle required.
