Main duties of the job As a Registered Nurse with current NMC registration you will have significant post qualification specialist experience in the field of Respiratory Disease.
With strong communication and organisational skills and a flexible approach, you will work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes.
You will be able to evidence Band 7 leadership experience, contribution or potential, within a community setting.
You will demonstrate the ability to effectively and efficiently assess, plan, provide and evaluate evidence based nursing care for patients.
You will demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.
You will have a commitment to professional development and contribute to a culture of continuous improvement.

To assess new service user referrals and care/support needs.
Assesses service and users and formulates, develops, implements and evaluates evidence based programmes of care with the aims of improving quality of life, reducing hospital admissions, empowering self-care and takes a lead in specialist area/complex caseload and providing expert advice and support to others.
The specialist care/support plans may include the following: Promoting independence/self-management and maintaining safety Increased functional ability Assessment/treatment/management and implementation of care for individuals Interventions in disease/condition management Prevention and reducing of health inequalities Admission avoidance Post hospital discharge review Case management/treatment/care plans Palliative and End of Life planning Drug prescribing and titration.
Specialised interventions around specific long term conditions and clinical need Improvement in health and wellbeing including mental health Ability to work without supervision or direction Person Specification Qualifications & Training Essential Degree in Nursing or equivalent Registration with NMC V300 nurse prescribing or completed within first 12 months of obtaining the position Mentorship qualification such as Practice Assessor or ENB 998/997 Relevant specialist post graduate (Masters) qualification and/or professionally recognised clinical course(s) in specialist area or significant equivalent experience with considerable evidence of professional development.
Ability to interpret specialised respiratory reports Blood gas analysis and venepuncture Desirable Advanced or intermediate life support qualification Further clinical study relevant to clinical practice such as management of long term conditions Experience Essential Significant period of post registration experience in clinical specialism Supporting service users, families and carers in dealing with respiratory disease and its associated complications Experience of negotiation and conflict management in clinical situations Experience of leading clinical audit in specialism Supervision of more junior members of a team Presentation skills Desirable Involvement in service improvement
