We are seeking an enthusiastic Band 3 Rehabilitation Support Worker to join our Community Enhanced Home Based Rehabilitation Service.
You will work closely with Physiotherapists and Occupational Therapists to help patients regain independence and wellbeing following recent hospital admissions due to illness or falls.
Main duties of the job The Rehabilitation Support Worker (RSW) functions as part of the Multi-Disciplinary Team to provide a seamless service to a group of patients under the guidance and supervision of an Allied Health Professional.The RSW will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice.The RSW will promote optimum independence and individualised patient-centred care to a delegated caseload.The RSW will also support the team to deliver against key performance indicators and maintain quality of the service About us Just as we care about our patients' well-being, we care about yours!

Person Specification Experience Essential Experience of working in a health or social care setting.
Experience of working as part of a team Experience with the use of Microsoft office and a variety of databases Desirable Demonstrable understanding of the roles of members of the MDT Experience as a therapy assistant Experience with the NHS patient information database (RIO) Skills Essential Good written and spoken communication skills Demonstrate understanding of the role of a rehabilitation assistant.
Demonstrate an acceptable level of numeracy and literacy An awareness of risk management and issues relating to working in people's homes.
Knowledge of multi- agency services available and processes for referral Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being.
Knowledge of the NHS Desirable Knowledge of local health needs strategies Qualifications Essential Higher Professional Diploma in Health and Wellbeing (NVQ - level 2 / 3) or equivalent Willingness to undertake further training as appropriate Desirable GCSE grade 9 - 4 or equivalent in English and Maths.
