Job summary The post holder will work primarily alongside the team's specialist Neuro Physiotherapist; but on occasion support with therapy interventions under the supervision of the occupational therapist, clinical psychologist, dietitian and speech & language therapist.
This post will allow candidates, who meet the person specification, to develop multi-disciplinary skills in the field of neuro-rehabilitation through gaining further knowledge and competencies.
Main duties of the job The role of the Assistant Practitioner is to deliver a high quality, caring andcompassionate patient care.
The post holder will be undertake assessment,implementation and evaluation of patient care needs for a defined group ofpatients under the indirect supervision of a registered professional in theirspecialist area.
The post holders are required to work as part of a team committed to delivering and improving a high standard of therapeutic care within South Warwickshire NHS Foundation Trust.

Clinical Practice Take responsibility for care and welfare of a defined caseload ofpatients/clients within your area of work (Community Multiple Sclerosis) Continuously assess, implement and evaluate treatment, using appropriateprogrammes / methods of care agreed by a registered professional.
To report to the registered professional any patient who gives cause forconcern Never undertake anything beyond your own level of competency.
To ensure delivery of high quality care in accordance with Trust policies,procedures and guidelines.
To accurately document all care delivered to patients, maintainingconfidentiality at all times.
Participate in activities to improve the quality, productivity and effectivenessof care in response to local and national policies and initiatives To adhere to Infection Prevention and Control policies relevant to the areaof work, demonstrating an understanding of cross infection.
To support relatives as required demonstrating compassion and empathy.
Provide supervision and support to junior staff as required Communications and Interpersonal Relationships Providing assistance, support and information where required, in patientsown homes, identifying individual needs and utilising a range of agreedcommunication mediums.
Always introduce yourself in line with the Trust privacy and dignity promisesand the principles of good customer service.
Communicate effectively and professionally with the multi-disciplinary team to ensure that patient care is well planned, coordinated and communicated.
Contribute to the reporting, handover and documentation of patient caremaintaining confidentiality at all times.
Use the Trust information support systems and other computer systems asrequired by the clinical area.
Team Working To act in a professional manner, and as a role model, at all times, leadingby example to junior members of the team Support and cooperate with the multidisciplinary team to ensure that allstatutory and Trust regulations are complied with.
Participate in research and audit as required by the multidisciplinary teamto ensure standards of patient care are monitored and improved.
Education, Training and Personal Development To ensure attendance at mandatory training sessions infection control,resuscitation and moving and handling training.
To complete the Trusts Clinical Support Worker Induction programme To successfully complete the Care Certificate within 12 weeks ofcommencing employment To ensure maintenance and development of individual competenceknowledge and skill As a competent practitioner, teach and provide supervision to appropriatejunior staff.
To participate in and undertake appropriate training and developmentactivities including IT and clinical skills Maintain up to date knowledge of changes in policies and procedures relevant to the post.
Person Specification Qualifications Essential Obtained an educational level of GCSE level A - C English and Maths or equivalent Minimum Foundation Degree in Health and Social Care or equivalent higher apprenticeship Experience Essential Significant experience in delivering care and working in a healthcare, people centred environment.
Experience working as part of a team.
Desirable Experience in supervising others at work Skills Essential Evidence of competence in patient assessment skills Evidence of specialist extended skills and competencies Evidence of regular updates and utilisation of specialist skills and competencies Able to implement and evaluate treatment Able to recognise when escalation to a registered professional is required Able to delegate work appropriately Good written, verbal and non-verbal communication skills Competent IT skills Able to present information, written and orally in an accurate clear and logical manner Ability to organise and prioritise workload Able to work autonomously Ability to write accurate and legibly in patient notes Desirable Experience of supervising others Personal Qualities Essential Able to work as part of a team.
