Main duties of the job We deliver a family centred service, which enables individuals, families, and communities to achieve their optimum physical, psychological, educational, and emotional wellbeing through a public health approach.
We are excited to be involved in the new Family Hub model which will continue to build on our partnership working with other agencies including Early Years teams, Early Help, Housing and Social Care.
We work within a variety of settings such as clinics, nurseries and family homes, no two days are the same.

Job description Job responsibilities Under the supervision of the Health Visitor or Senior Childrens and Young PeoplesNurse, effectively manage a defined caseload as allocated nurse ensuring earlyintervention and reducing health inequalities.
Delivering careprogrammes/packages that promote a child or young person's health, safety anddevelopment and that support a child or young person's individual health needs thatare outside of the responsibility of specialist health care services.
This will includedelivery of certain aspects of the Healthy Child Programme such as Antenatalgroups and health promotion and core contacts but only where training andcompetencies are completed.Work with the childrens health service developing and maintaining appropriaterelationships with individuals, families, service users, stakeholders, and support anintegrated provision of services to children, young people, and their families as part ofthe Council's Early Help approachWhere relevant, liaise with other members of the 0-19 (25, SEND) children's servicesworkforce , early help, social care and outside agencies to identify/agree how theneeds of the child or young person are best met and when/where additional support isrequired.Under the supervision of the Health Visitor or Senior Childrens and Young PeoplesNurse, deliver specific health interventions/care programmes based on individualhealth needs assessments to children and young people on a child protection plan,child in need, TAC and ESCO relevant to the child's individual needs and act inaccordance with safeguarding policy and procedures.
Attending child protectionconferences, core groups, child in need meetings as allocated, with support andguidance from the Health Visitor or Senior Childrens and Young Peoples Nurse.Utilise specialist clinical skills using evidence-based practice to identify any risks andneeds in protecting children and adhere to agreed safeguarding policies andprocedures of Childrens Services Directorate and Lincolnshire Safeguarding ChildrenPartnership / Lincolnshire Domestic Abuse Partnership .Under the supervision of the Health Visitor or Senior Childrens and Young PeoplesNurse act as lead professional where relevant and the child or young person is inreceipt of a children and young people's health intervention/care programme.Provide mentorship and community experience for pre-registration students.
The rolewill also provide 1-1 support, guidance and supervision for Family Health Workerswhere required.
This will also include the provision of appraisals for Family HealthWorkers.Support managers within Children's Services in the delivery of operational objectives,plans and targets and to review and monitor these to improve team / directorate policyby operating effectively as a member of the Children's Services Directorate.Participate in the development of quality intervention and excellent standards throughquality audits.Act as a role model to others helping them to manage uncertainty and to respondpositively and creatively to change.Work flexibly as required across the core hours of the service 8-8pm includingweekendsThe post holder will maintain their own expert level of clinical competence andmaintain a professional profile and Nursing and Midwifery Council revalidationrequirements.
The post holder will receive clinical supervision and safeguardingsupervision to support their own practice.As an employee, create a positive image of the County Council to support anintegrated provision of services to children, young people and their families as partof the Council's Early Help approach and deliver excellent customer service,incorporating the Councils equality and diversity objectives and supporting thecouncil to achieve best practice in all it delivers.Maintain and record accurate contemporaneous documentation regardinginteractions between service users, colleagues and other agencies in the appropriatelegal records and formats.Always utilise and apply appropriate communication skills with service users.
WhereEnglish is not the first language or understanding is limited or required in differentmode to seek advice regarding methods of communication to support the individual translator services or resources for the visually or hearing impaired.

Qualifications Essential Registered Childrens Nurse (or other registered nursing domain with childrens experience).
Current registration with NMC Experience Essential Post registration experience in Childrens Health / pediatrics.
Experience and knowledge of current safeguarding children policy and process.
Knowledge of health issues affecting infants, children and young people.
Knowledge of clinical governance including CQC compliance, audit and embedding lessons learned.
