Job summary We are focus on medical patients safety experience throughout the admission, transfer and discharge.
We are currently looking for a Band 5 Registered nurse to join our team and the successful candidate will progress their nursing skills in nursing assessment , admission, treatment of various medical pathways, transfer and discharge process.
Main duties of the job Communicate effectively with patients and their relatives, ensuring accurate and seamless information is conveyed and documentedEnsure that accurate and legible patient records are kept in line with professional and legal requirementsAct as the identified nurse for critically ill patients by assessing, planning, implementing and evaluating their care.Participate in taking an active role in planning effective, timely, and safe dischargesWork to eliminate all avoidable infection by acting as a role model in infection prevention and control prevention to all staff, visitors and patientsProvide training and support to new and junior staff in accordance with Trust policies and proceduresIdentify and minimise clinical and non-clinical risk to minimise risk of harm to patients, staff and visitorsInvolve patients in their care and ensure their understanding of treatment plan and care options before gaining informed consent for care delivery and interventionsEnsure that Privacy and Dignity of patients is maintained at all times and work to minimise mixing of male and female patients and sharing of bathroom / toilet facilitiesParticipate in department meetings when required.Ensure compliance with professional expectations as outlined in the NMC Code of Professional ConductTo participate in an annual appraisal with relevant reviews and to follow your personal development plan as agreed with your line manager About us We are one of the largest hospital trusts in England, with five hospitals and community clinics serving a local population of around 800,000 people.
