Registered Nurse For Complex Care (Domiciliary Care)

We are seeking a skilled and compassionate Registered Nurse to join our Complex Care Domiciliary Team.
In this role, you will provide specialized care to customers with complex medical conditions, ensuring they receive the best possible care in their own homes.
You will work closely with a multidisciplinary team to develop and implement individualised care plans, monitor customer progress, and adjust care as needed.
Key Responsibilities: Provide direct nursing care to customers with complex health conditions in their homes.
Develop, implement, and regularly review individualized care plans in collaboration with patients, families, and the healthcare team.
Administer medications, manage medical equipment, and perform clinical procedures as required.
Monitor customer conditions, recognizing and responding to changes in health status.
Provide education and support to patients and their families to help them manage their conditions effectively.
Maintain accurate and detailed records of patient care and progress.
Ensure compliance with all relevant healthcare regulations and standards.
Requirements: Registered Nurse with a current UK Nursing license and PIN Experience in complex care and injuries, critical care, or similar settings is preferred.
Strong clinical skills with the ability to manage a variety of complex medical conditions.
Excellent communication and interpersonal skills.
Compassionate, customer -focused approach to care.
Ability to work independently and as part of a multidisciplinary team.
Valid driver's license and reliable transportation.
